ACT Co-Occurring Disorder Specialist
Position Description
Under the supervision of the ACT Team Leader, the Co-Occurring Disorder Specialist will provide substance abuse services to clients with co-occurring severe and persistent mental illness and substance abuse disorders as part of the ACT Team. Incumbent will participate in collaborative client assessment and treatment planning. Incumbent will provide services in the client’s home and in the community to support integration and stabilization. All services are provided as part of a comprehensive treatment team. This position involves rotating on-call duty overnights and holidays.
Minimum Qualifications:
· Bachelors degree in a relevant field with professional, clinical experience in the provision of substance abuse treatment or a combination of education, training, and experience indicating the possession of essential knowledge, skills, and abilities.
· Certified substance abuse counselor (CSAC), supervisee (CSAC-S), or assistant (CSAC-A) with training or experience working with adults with co-occurring serious mental illness and substance use disorder
· A valid Virginia driver’s license and the ability to travel independently to a consumer’s residence, work site and/or hospital setting required.
· The ability to be on-call and provide emergency response required.
· Ability to communicate and consult effectively with other healthcare professionals.
Duties and Responsibilities:
1. Assists in the development, implementation, and maintenance of ACT team services.
2. Performs diagnostic assessments, determines client service needs, and develops treatment/service in coordination with ACT Team.
3. Provides direct outpatient client treatment services in various modes, including individual, group, and family therapy.
4. Develops and maintains substance abuse group services, including monthly ACT SA outpatient group that provides education treatment and after-care support for substance abuse clients.
5. Provides client-centered treatment utilizing various modalities including motivational enhancement and dual-disorder treatment to encourage abstinence and sustained recovery.
6. Assesses clients in need of inpatient residential/detox treatment. Coordinates admission and discharge from inpatient treatment.
7. Collaborates and liaisons with law enforcement officers, local hospitals, medical providers, and others in the planning, provision, and evaluation of these services.
8. Participates in daily ACT treatment team meetings to discuss client treatment goal and needs.
9. Collaborates with other members of ACT Team to provide comprehensive mental health and supportive services to clients.
10. Produces client related service records and maintains accurate and up-to-date documentation consistent with agency policies and procedures.
11. Receives clinical and procedural guidance from ACT Team Leader.
12. Required to provide back-up ACT coverage on the rotational schedule as appropriate to hours worked.
13. Performs other duties as assigned by supervisor which are consistent with the position and in compliance with agency policies and procedures.
14. Displays commitment to the goals of ACT, the ACT philosophy and the team’s response to the needs of the consumer.
15. Serves as a member of the ACT team providing on-going case management, medication monitoring, life-skill development, supportive counseling, crisis intervention, and/or specialized services consistent with training/certification to ACT consumers as warranted and/or requested.
16. Establishes and maintains a treatment relationship with the consumer providing face-to-face services to the consumer in the home, the community, and with other agencies and family as appropriate.
17. Participates in the development of individualized treatment/recovery/rehabilitation/wellness plans, overseeing the identification of resource development and delivery of appropriate clinical specialty services.
18. Coordinates and provides ACT Team training and development in areas of clinical and/or management specialty.
19. Provide direct and emergency back-up service support to members of the ACT team.
20. Provides available on-call services on a 24-hour 7-day per week basis.
21. Responsible for accurate and complete documentation of provided services in accordance with established policy guidelines for record keeping.
22. Participates in on-going supervision and/or program development with the Team Leader which may consist of field visits to the consumer's residence, work site, family, etc.
23. Participates in the ACT Team's daily planning and coordination meetings, treatment planning conference meetings, etc.
Job Type: Full-time
Pay: $48,000.00 - $55,000.00 per year
