Example of Position Responsibilities:
-
Completes healthcare disease management goals and inserts into comprehensive treatment plan in partnership with behavioral health case manager (in partnership with team).

Developing contracts or Memorandums of Understanding (MOUs) with regional hospitals or system(s) to ensure a formalized structure for transitional care planning, to include communication of inpatient admissions and discharges of Health Home Clients.

Oral/Nasal/ Injection Medication Administration, documentation, and monitoring as needed.
-
Assists with prior authorizations for medications.
-
Supervises Wellness coach(es); conducts interviews with clients to discuss wellness goals.
-
Completion of the Health Risk Appraisal (HRA)- Wellness plan development; A systemic approach to collecting information from individuals that identifies risk factors, provides individualized feedback, and links the person with at least one intervention to promote health, sustain function and/or prevent disease.

Maintaining a mutual awareness and collaboration to identify individuals seeking emergency department services that may benefit from connection with a Health Home Site.
Other Duties: Perform other duties as assigned by Supervisor.
Physical Requirements: Must be able to move from location to location, bending, stooping, lifting 25 pounds, communicate clearly, prepare written communications, present information to other employees, process complex information within established procedures/programs, enter data to various programs. Must be able to climb stairs and access non-handicapped equipped residences/facilities.
