Under supervision and direction of a registered nurse, nurse practitioner, or physician, is responsible for following doctor/nurse instructions, obtaining client’s accurate health and substance abuse history, performing physical assessments of clients undergoing detoxification, performing accurate vital signs, and proficient in making medications available to all clients.
DUTIES:
1. Obtains health history and substance abuse history by interviewing client and/or relative.
2. Takes and records client's vital signs.
3. Performs accurate health assessment for all new clients, including assessing for possible need of detoxification.
4. Delivering care to patients utilizing the nursing process of assessment, planning and intervention.
5. Medications data entry and ensuring that medications are available to the client. Verifies that medication is made available to the right client.
6. Interacts with patients, significant others, and other health team members while maintaining professional standards of nursing.
7. Documents accurate and ongoing care using the appropriate required documentation (logs, reports, etc.)
8. Communicates and collaborates with team members to ensure patients well-being as evidenced by staff/client feedback.
9. Inventories and maintains appropriate medical supplies.
10. Performs weekly inventory of all medications in the facility.
11. Calls in prescriptions to appropriate pharmacies, on the order of the medical provider.
12. Interacts and coordinates with pharmacies and other medical providers.
13. Requests medical records for clients from other providers within HIPAA guidelines.
14. Performs tuberculosis tests and updates the client's health questionnaire for all new clients.
15. Performs appropriate mental status exams using PHQ/GAD screening tools.
16. Attends and participates in medical staff meetings.
17. Provides MAT service consultations for clients.
18. Provides weekly updates on MAT clients to the medical team.
19. Identifies and serves as medical case manager for high acuity clients, and coordinates with the medical team of any new or ongoing problems or concerns.
20. Teaches and runs health groups.
21. Keeps equipment operating by following operating instructions; troubleshooting
Break-downs; maintaining equipment supplies; performing preventative maintenance; calling for repairs
22. Per HIPAA, CFR 42, Part 2, and other confidentiality laws, ensures confidentiality of client information
23. Adheres to professional standards, accreditation, policies and procedures per federal, state, county and local requirements.
24. Participates in educational opportunities; reading professional publications; participating in professional organizations; maintaining licensure.
25. Teaches clients about their health issues and any medications they may be taking.
26. Demonstrate appropriate problem solving skills
27. Other duties, as may be assigned.

Pay: $27.00 - $37.00 per hour
