Welcome Health is an innovative medical group that delivers home-based primary care directly to older adults in the greater Los Angeles area. Founded in the science and art of geriatric medicine, the team at Welcome Health believes that the best primary care is anchored on what matters most to each person. Our care model recognizes the unique needs of older adults. In addition to preventative care and chronic disease management, we focus on aging-related issues that significantly impact the quality of life of our seniors, such as mobility limitations, cognitive impairment, incontinence issues, and polypharmacy. In our physician-led care model, medical assistants (or patient navigators) work closely with physicians and other members of the team to implement the care plan. Because we care for patients who have difficulty getting to the doctor, the work will be either in the patient's home or remotely from your home. We are a group of people who believe that seniors and their families have earned the right to respectful, accessible, and comprehensive primary care. Join us!
