We are seeking a full-time Mobile Outreach Response Engagement Stabilization - Qualified Professional to provide the following services:
The Mobile Outreach Response Engagement Stabilization (MORES) Qualified Professional (QP) provides services to Mental Health and Substance Use individuals and is intended to prevent placement into more restrictive settings, unnecessary court involvement, and decline in functioning. This position provides mobile response services 24/7/365, as needed. Also, face-to-face responses to better address the needs of children and families in their homes, schools, and communities. Response interventions include crisis intervention and de-escalation, counseling, behavioral assistance, skill building, medication management, and/or caregiver and youth engagement/support/stabilization services, and linkage to ongoing care.
- Available to respond to individuals and co-workers 24/7/365, as needed.
- Responds within 45 minutes of individual/guardian outreach requests. 80% of the time, the response will occur face-to-face when a child is experiencing an emotional or behavioral crisis unless a deferred response is requested by the parent/caregiver.
- Continuously assess and reassess the individual’s need for services.
- Serves as a member of the MORES team and assists and/or coordinates, as assigned, in planning and implementing community engagement and other such activities related to the individual’s therapeutic goals and mental health needs.
- Assists in planning, scheduling, and service delivery with other internal staff or external agencies.
- Works with clients in reviewing, applying, and achieving their goals as indicated in the Person-Centered Plan.
- Utilizes community-based activities as a means to deliver clinically appropriate behavioral/therapeutic interventions, crisis interventions, and rehabilitative and psycho-educational activities.
- Informs the individual/guardian about benefits, community resources, and services, and assist in accessing and monitoring such benefits and services.
- Assists in all service delivery as formally planned with the clinical team.
