JOB TITLE: Financial Case Manager

STATUS: Hourly 37.5 hours weekly, full-time
HOURLY RATE: $21.50 to $22.50
SUMMARY
Under general direction, provides financial case management services to adults 60 years of age and older, primarily within the Lifespan Guardianship Program. This active pace position is responsible for maintaining an average caseload between 20-30 clients and is responsible for general management of financial affairs including bill paying and helping to plan and maintain long-term financial security of clients. The Financial Case Manager is responsible for processing Social Security and Medicaid applications along with other benefit-related applications. The Financial Case Manager is responsible for compiling the annual accountings to the court and occasionally will meet with clients, in their homes, the office, or other community-based establishment: hospitals, nursing homes or bank settings. The Financial Case Manager is also responsible for rotating on-call emergency phone coverage.
DUTIES AND RESPONSIBILITIES
1.0 Provide appropriate financial case management and budgeting services to ensure clients are served properly.
1.1 Prepare and compile comprehensive personal & financial court reports; monthly activity reports; and summary reports. Collect data and statistics as required.
1.2 Attend court hearings as appropriate.
1.3 Manage and administer client financial matters.
1.4 Complete comprehensive financial assessments to determine needs.
1.5 Responsible for estate management and sound financial management to ensure highest quality of care of property.
1.6 Create and plan appropriate actions that will assist client in becoming as financially secure as possible.
1.7 Marshall and consolidate assets.
1.8 Provide budgeting services.
1.9 Assist with financial decision making.
1.10 Reassess client needs and revises plans as needed.
1.11 Advocate with financial, creditor and other individuals/institutions on behalf of client.
1.12 Appropriately bill clients for services rendered.
1.13 Maintain and manage all records and reports to ensure accuracy and compliance with policies and procedures.
1.14 Attend client service meetings when needed.
1.15 Monitor personal financial needs plan and amend as needed to address client level of care needs.
1.16 Attend regular Guardianship team meetings to review cases and make financial decisions on behalf of clients.
1.17 Document services delivered.
1.18 Act as primary liaison to families, advocates, and other service providers
2.0 Maintains up to date professional knowledge of the community services entitlement benefits, and competencies through reading, workshops, and conferences.
2.1 Participate in all Lifespan staff meetings and training courses as appropriate.
2.2 Maintain Kkowledge of eligibility requirements for financial, and social services programs.
3.0 Other duties as assigned by supervisor.
Requirements:
QUALIFICATIONS
EDUCATION: Associate in applied sciences required. Bachelor’s degree in business, Finance, Human Services or equivalent Preferred.
EXPERIENCE: Three years of experience in financial services, banking, accounting, community resources.
Completion of Certificate of Gerontology program within first 18 months or equivalent.
Computer experience required (Office 365, Word, Excel, SEM, web research)
PHYSICAL AND MENTAL REQUIREMENTS
- 15% of the work is community based: must be able to meet with individuals in their home environment and in a variety of community settings including hospital and nursing home environments.
- 85% of the work is performed at a desk or workstation
- Must be able to work in an open office environment
- Must demonstrate verbal, written, organizational and leadership skills
- Must be able to maintain composure under difficult and challenging situations
- Must be able to work in multi-cultural situations
- Must have valid NYS driver license and reliable transportation
- Must demonstrate working knowledge of Excel, Access, Word, Outlook, internet web-based programs and web applications
- Normal daily walking and sitting.
- Frequent computer usage.

If working less than 37.5 hours, accrued time is pro-rated.
