Job Overview:
Northside long term care facility seeking a detail-oriented MDS Coordinator to join our team. The MDS Coordinator will be responsible for ensuring accurate and timely completion of the Minimum Data Set (MDS) assessments and care plans for our patients.
Duties:
- Conduct comprehensive assessments of patients' needs
- Develop individualized care plans based on assessment findings
- Ensure compliance with regulatory standards and guidelines
- Collaborate with healthcare providers to optimize patient care
- Utilize clinical documentation improvement strategies
- Monitor and update patient records regularly
**Qualifications:**
- Bachelor's degree in Nursing or related field preferred
- Active RN license
- Previous experience in case management and utilization management
- Knowledge of Medicare regulations and standards
- Familiarity with medical terminology, ICD-9 coding, and anatomy
- Experience in pediatrics or nursing home settings is a plus
- Strong clinical documentation skills
