Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. We are seeking qualified candidates for the position of Case Manager. The position has responsibility for providing engagement and case management support including home-based visitations and other services within the Philadelphia community. Approximately 75% of work is in the community. Interact with members of the multidisciplinary team to ensure that participants receive the individualized services designed to help them successfully maintain their permanent housing. To provide services in compliance with contract and Agency regulations. Other responsibilities include the following:
- Actively participates as a member of a multidisciplinary team of professionals focused on the provision of comprehensive and flexible services to participants. Expected to contribute to a participant-centered program culture that is outcome-oriented and committed to quality improvement.
- Conducts and coordinates outreach activities designed to locate and engage TCMU authorized participants.
- Submit accurate and timely documentation and related information in accordance to TCMU/CBH/HH standards. Document all contacts, movement, growth, transfers and overall progression through the system, in accordance with funding source regulations. Expected to consistently meet daily productivity standards as required by the funder and department.
- Provide clinical services to participants as needed in their natural environment or in their program housing or assist participants to access mainstream clinical services.
- Participate in the resolution of crisis situations; assist with identifying key individuals and resources that can provide direct support and/or immediate intervention during a crisis situation. In conjunction with other staff members, provides on-call crisis intervention covering nighttime and weekend hours.
- Works with participants to develop, implement and monitor their personal goal plans to enable them to stabilize and retain housing on a permanent basis.
- Assist clients in the housing process, including procurement of necessary documentation, completion of housing applications, preparation for move-in, and support after move-in.
Qualifications:• Ideal candidate will have a Bachelor’s degree with concentration in sociology, social work, psychology, gerontology, anthropology, criminal justice, theology, nursing, counseling or education.
- Must be able to communicate both verbally and in written form with every level of the behavioral health system and maintain effective professional relationships with associates in the Agency/community at large.
- Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for participant rights and personal preference in treatment are essential.
- Ability to work independently and as part of a focused team.
- Working knowledge of Outlook, MSWord and Excel.
- Ability to maintain daily productivity standards.
- Fee-for-service and electronic health record experience strongly preferred.
- Valid driver's license with an acceptable driving record required.
Scope:
- Frequent talking and listening.
- Requires the ability to lift 25 pounds, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with participants.
- Possess manual dexterity and fine motor skills.
- Must be available for local travel and possess a valid driver’s license with an acceptable driving record.
- Must carry a mobile phone during scheduled working hours and scheduled on-call services.
- As essential personnel, may be required to report to work during emergencies including inclement weather.
- Requires crisis intervention and availability to respond in emergency situations.
