UConn Health Office of Patient Experience is seeking a Staff Nurse - Post Discharge Phone Calls. This position is accountable for contacting patients after discharge to review and assist with discharge instructions and assess patient experience. This full-time position will work in the Office of Patient Experience located in Farmington, CT.

SCHEDULE: Full time, 40 hours per week, 8:00 a.m. to 4:30 p.m. 30-minute unpaid meal break.
EXAMPLES OF DUTIES:
Reviews discharge plan with patients, addressing post-discharge concerns and questions.
Establishes linkages to UConn Health and community resources.
Accesses, reviews, and interprets electronic medical record documentation.
Applies service recovery techniques and escalates concerns appropriately.
Collaborates with Patient Relations, Clinical Managers, and others to resolve concerns/complaints/grievances.
Responds to patient/family complaints/grievances and triages appropriately.
Ensures calls are documented accurately and timely.
Leverages databases for record keeping and data analysis.
MINIMUM QUALIFICATIONS:
KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge of the principles and practices of nursing
Knowledge of relevant health center policies and procedures
Considerable interpersonal skills
Oral and written communication skills
Ability to establish priorities and manage effectively.

Bachelor's degree in Nursing and four (4) years of inpatient clinical experience.
Experience conducting post-discharge phone calls and utilizing related call management systems.
Experience reviewing electronic patient medical record, i.e. EPIC
