Position Summary
Parc Center for Disabilities’ Bert Muller Home (BMH) is a long-term care (intermediate care) facility for individuals with intellectual and developmental disabilities. We operate 24/7 with nurses, employees, and supervision. LPNs are responsible for providing optimal healthcare for our residents. Nurses must interface effectively with all other disciplines to ensure the provision of comprehensive, seamless, integrated services.
Under the general direction of the Medical Coordinator, the Licensed Practical Nurse (LPN) is responsible for administration of medications and related medical treatments, documenting accurate and appropriate information in medical records, assessing residents for any changes in physical, mental, or neurological status; provide first aid in accidents or illness; and providing health related services to residents as required.

Essential Responsibilities

Administration of medications and treatments per physician orders, ICF regulations and program policies.

Provide legible, timely and accurate documentation of all procedures and events – including but not limited to: MAR, nurse’s notes, Physician orders, ordering, infection control & routine communication.

Assess residents as needed for signs and symptoms of illness, infections, disease, injury, adverse reactions or change in physical, mental, or neurological status. Provide or arrange physician/dental follow up, first aid or emergency treatment as needed. Report and document all concerns promptly to the appropriate program positions.

Ensure that proper notifications are made as necessary, and Report concerns promptly.

Document accurate information in a timely manner in nursing notes. Responsible for charting residents that are on antibiotics, post incident, new admission, readmission, or any special charting that needs to be done per shift (incidents, medical concerns, ER visits, etc.), medical appointments and new orders.

Ensure that physician orders are accurately recorded according to laws and nursing protocol. Ensure that medications/treatments are accurately and promptly ordered (and re-ordered) from the pharmacy and that prescribed procedures are completed as ordered. Provide immediate follow-up and action as needed to ensure correction of any error or delay in receipt of medications or treatments.

Provide residents with health-related services, including but not limited to monitoring of routine vital signs and weights. Record weights and other required data according to program procedures.

Provide positive interaction with residents during nursing procedures, medication pass, client’s leisure time, etc.

Provide education and on the spot training to DSPs and supervisors as situations and opportunities arise.

Maintain infection control and safety measures in the nursing office and throughout the building.

Submit assigned assessments & reports promptly, adhering to due dates.

Assist with physician clinic services as needed.

Participate in all required meetings and training to enhance nursing skills and required by Parc Center for Disabilities.

Other duties as assigned.
Requirements:
Education, Training and Experience Requirements
-
Minimum 2 years’ experience in nursing home, assisted living facility (ALF), long term care (ICF) is required.

Prior work experience with people with intellectual or developmental disabilities highly preferred.

High school diploma or equivalent required.
-
Must be currently licensed by the Florida State Board of Nursing

Must have CPR, and First Aid certifications
-
Valid Florida Drivers’ License with acceptable driving record.

Be physically, visually, and auditorily aware of the cognitive, social, and physical needs of our participants.
Knowledge, Skills & Abilities

Ability to communicate effectively in English both verbally and in writing.

Ability to read and follow instructions and safety precautions.

Physical demands such as repetitive motion, standing, bending, kneeling, walking, and lifting for extended periods of time.

Schedule:

Night shift – 9:30pm – 5:30am
