JOB DESCRIPTION
I. TITLE: COMMUNITY INCLUSION FACILITATOR
II. WORKING HOURS: 8:00 A.M. TO 4:00 P.M. MONDAY THRU FRIDAY
III. IMMEDIATE SUPERVISOR: PROGRAM MANAGER
IV. JOB SUMMARY:
Working in conjunction with the Program Manager and Activities Coordinator, the Community
Inclusion Facilitator will assist in directing, supervising and coordinating activities within the areas of assigned responsibility. The Community Inclusion Facilitator will work alongside program staff to deliver appropriate services to our consumers on an individualized and person-centered approach, as well as carry out the mission and goals of AR&C Long Beach and that of our day programs. Will provide on an as needed basis attendant care and assistance in toileting and feeding for assigned program participants.
V. JOB DUTIES: ( Bold: Essential Function) (Non-bolded: Ancillary or Marginal Function)
1. Cooperate with Program Manager and Activities Coordinator in implementing individual or
group activities designed to engage and benefit our consumers either as a group or
individually.
2. Meet with Activities Coordinator as scheduled by the Program Manager to foster
understanding of the benefits of the selected activities as they relate to individual needs of
program participants.
3. Document progress of program participants individualized objectives or activities on a daily
basis.
4. Carry out specific duties as designated by the Activities Coordinator or Program Manager,
including the following tasks, but not always limited to toileting, hygiene, feeding, clean up,
chaperoning, bus loading and unloading, and other tasks needed to prepare for the scheduled
daily activities.
5. Use personal initiative, resourcefulness, enthusiasm and good judgment in implementing individual or group activities designed to engage and benefit consumers as a group or individually.
6. Interact with all program participants in a positive, supportive manner, which fosters respect for all individuals.
7. Maintain confidentiality of information about program participants.
8. Cooperate with other staff persons in developing and implementing overall training activities for the program.
9. Provide leadership and direction to program participants during all activities, which includes
constant supervision of program participants as well as modeling and enforcing all safety
procedures.
10. Attendance at and participation in staff meetings.
11. Adhere to all rules and regulations and policies of the organization and its programs.
12. Provide ongoing direct assistance to program participants in toileting and feeding as
necessary.

13. Assist in maintaining AR&C Long Beach vehicles by regular inspection and identifying problems and reporting them accordingly.
14. Inform Program Manager/ Activities Coordinator and other staff of any difficulties or special needs of program participants related to toileting and feeding.
VI. ESSENTIAL PHYSICAL REQUIREMENTS:
1. Must be able to bend or kneel to perform an activity such as:
- Administering first aid.
- Assisting consumers.
2. Must have physical ability to change consumers in case of an accident.
3. With assistance, must be able to transfer consumers from wheelchair to toilet or chair.
4. Must be able to assist consumers who have walking difficulties by allowing consumers to lean
on them for support.
5. Must be able to stand and walk outdoors up to 80% of the work day.
6. Requires the ability to lift, move, and push items which may weigh up to 50 pounds.
7. Must have visual acuity to perform an activity such as:
- Preparing and analyzing data and figures, transcribing, viewing a computer terminal,
extensive reading.
8. Must be able to effectively communicate with consumers and staff and accurately exchange
information in all situations.
9. Must be able to identify and detect safety hazards, changes in consumer behavior, and
determine appropriate action(s) needed.
10. Must possess problem solving and decision-making skills that may impact the program.
11. Must be able to remain stationary/sitting at desk with computer while compiling data, creating
reports, communicating online with other agencies associated with the program, and when
necessary, researching information pertaining to the service delivery system and its impact on
the Program.
VII. POSITION(S) SUPERVISED:
Program participants as assigned.
VIII. QUALIFICATIONS:
High school diploma or its equivalent with a minimum of one (1) year paid experience working with individuals with intellectual disabilities in a center-based or community-integrated setting. Must possess a current Drivers License and be able to drive company vehicle.
