Position Summary:
The Community Hub Data Analyst position is responsible for connecting data to the Greater Health Now (GHN) Community Care Hub initiatives. This position requires the ability to gather, analyze, and interpret community hub data, translating insights into actionable strategies that enhance projects and initiative outcomes and support informed decision-making. The specialist will work closely with various organizations, offering technical assistance and fostering collaboration to advance community hub analysis. Key responsibilities include developing data-driven solutions for population health improvement, creating and deploying tools to measure effectiveness, and predicting data trends to guide GHN's health equity initiatives.

Essential Job Functions
Data Collection and Integration:
- Gather data from diverse sources related to Community Care Hub projects, ensuring data accuracy and completeness.
- Integrate data from various platforms and systems to create comprehensive datasets for analysis.
Data Analysis:
- Conduct a thorough analysis of quantitative and qualitative data to identify trends, patterns, and insights relevant to the Community Care Hub projects.
- Utilize statistical methods and data visualization tools to present findings clearly and compellingly.
Performance Measurement:
- Develop and implement key performance indicators (KPIs) to assess the effectiveness of Community Care Hub initiatives.
- Track and report on project outcomes, highlighting areas of success and areas for improvement.
Reporting and Documentation:
- Generate regular reports summarizing data analysis results and presenting actionable recommendations.
- Document data analysis methodologies, ensuring transparency and reproducibility of results.
Collaboration:
- Collaborate with cross-functional teams, including project managers, healthcare professionals, and community stakeholders, to gather insights and ensure alignment with project goals.
- Provide data-driven support to inform decision-making processes and optimize project strategies.
Continuous Improvement:
- Identify opportunities for process improvement and contribute to the refinement of data collection and analysis procedures.
- Stay informed about industry best practices and emerging trends in data analysis.
Other:

Qualifications and Requirements
Education:
- High school diploma or GED, required
- Associate’s degree required
- Bachelor’s degree from an accredited college or university in data science, statistics, mathematics, or a related field, preferred
Experience:
- Minimum two years of experience in data analysis with a focus on project evaluation and impact assessment.
- Bilingual English /Spanish, preferred.
Knowledge, Skills and Abilities:

- Knowledge of healthcare regulations and community-based healthcare initiatives is a plus.
