Our agency is seeking dedicated and skilled Field Registered Nurses to join our team in our Houma location. As a Field RN, you will provide comprehensive nursing care to patients located within the surrounding cities and parishes of Houma in their homes, assess their health needs, develop care plans, follow provider orders, and coordinate with other healthcare professionals to ensure the best possible outcomes.
Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans.
- Perform routine Admits, Discharges, and skilled nursing visits when appropriate.
- Implement current nursing practice.
- Administer medications and treatments as prescribed by physicians or providers.
- Monitor patient progress and update care plans as needed.
- Educate patients and their families on health management and disease prevention.
- Collaborate with interdisciplinary teams to provide coordinated care.
- Conducts supervisory visits to agency home health aides and LPNs to evaluate their performance in skilled patient care.
- Maintain accurate and timely documentation of patient care activities.
- Participate in an on-call rotation to respond to patient needs outside regular business hours.
- Use equipment and supplies safely and efficiently.
- Fulfill additional performance responsibilities as assigned and necessary.
Qualifications:
- Valid RN license in the state of Louisiana, unencumbered.
- CPR/BLS Certification
- Vehicle and auto insurance
- Minimum of 1-2 years of nursing experience, preferably in home health or a related field.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Reliable transportation and a valid driver's license.
- Must demonstrate competence to render care independently and by home health regulations and nursing practice standards.
- Ability to work independently as well as within a team environment.
