CCBHC- MENTAL HEALTH CARE COORDINATOR
MISSION STATEMENT: Center Point, lnc.'s (CPINC) mission is to provide comprehensive social, educational, vocational, medical, psychological, housing, and rehabilitation services to combat social problems such as substance abuse, poverty, unemployment, and homelessness. CPINC offers rehabilitation and treatment services that interrupt the abusive cycles of psychological, social, and economic dislocation by providing critical training and support so that individuals and families can claim self-worth and dignity.
FUNCTION OF THE POSITION:
CPINC employees are required to model the agency's values as outlined in the Personnel Policies Manual. CPINC employees are required to maintain the Center Point's philosophy and standards of quality. It is the responsibility of every employee to adhere to all federal, state, and local authorities' rules and regulations. This position serves as the care coordinator of the multidisciplinary team, ensuring that the coordinated systems of care are responsive to the assessed needs of clients in a timely and comprehensive manner. The MH Care Coordinator ensures the integration of substance use and mental health disorder treatment and primary care services in a culturally competent, trauma-informed, ethical, legal, and moral manner within a safe and therapeutic environment consistent with applicable regulatory and accreditation standards. The MH Care Coordinator is an integral part of the clinical team and participates in delivering addiction and mental health treatment services to clients. The MH Care Coordinator also performs other duties to ensure overall program compliance and quality assurance within Center Point programs.
EDUCATION AND WORK EXPERIENCE:
- Licensed clinician in California (LMFT, LCSW, LPCC, etc.) Associate or Certified as a Counselor.
- license to be valid and in good standing with the California Board of Behavioral Sciences
- Knowledge of drug addiction/substance use and co-occurring disorders, abuse patterns, treatment methodologies, and medication-assisted treatment is required.
- Experience in data collection and analysis is necessary.
QUALIFICATIONS (KNOWLEDGE & SKILLS REQUIRED):
- Have a valid California Driver's license and maintain a satisfactory driving record.
- Strong use of the English language in verbal and written communication.
- Have a "can do" attitude and be a team player.
- Ability to work in a fast-paced environment and maintain poise under pressure.
- Be self-directed, motivated, resilient, and flexible.
- Ability to sit, walk, stand, bend, squat, climb, kneel, and twist intermittently or continuously.
- Ability to grasp, push, and pull objects such as files and file cabinet drawers and reach overhead.
- Ability to lift and carry up to 25 lbs.
