Summary:
The Care Manager is a Registered Nurse who manage all aspects of anticoagulation care. The RN functions under an RN protocol and has assistance from a Pharmacist Clinician. The job entails adjusting medication dosage based on the INR results, which can be obtained from a lab draw, point of care machine or by home monitoring. They collaborate with the Pharm Clinician to develop peri-procedural plans for medication interruption, responding to critical lab values and counseling patients on a full range of issues pertaining to anticoagulation. This includes signs and symptoms of hemorrhagic or thromboembolic complications, dietary interactions, alcohol consumption, medication interactions and how underlying medical conditions affect anticoagulation therapy. The Anticoagulation Clinic Nurse will assist anticoagulated patients both in the office and via telephone management to assist maintaining a safe and therapeutic dosing regimen of warfarin.

Registered Nurse (RN) - State of New Mexico or Compact State Nursing License required. BLS certification within 90 days of hire.
