OCES supports older adults and individuals with disabilities by providing information and coordination of services promoting healthy, safe lives for our consumers. We want to help them stay as active and engaged in their lives and in their own communities.
And we are growing! Due to promotions and growth, OCES is seeking several Registered Nurses (RN) to support our consumers in the community.
PART TIME - 20 HOURS PER WEEK.
What you'll do:
Assess and/or authorize services to be delivered in home by provider agencies to coordinate cost effective and quality service plans, which includes:

Performing in-home evaluation/clinical assessment to determine consumers' eligibility for programs and services according to procedures and guidelines.

Establishing, reviewing, and revising care plans as needed.

Working with the care management team to determine appropriate service options; promote cost-effective service substitutions; establish the frequency, scope, and duration of services; and jointly authorize service packages.
Facilitate successful discharge activities and other transition support activities as required for elders and people with disabilities to experience a smooth transition from facility to home, which includes:

Performing on-site evaluation/clinical assessment of consumers in nursing facilities to determine appropriateness of SNF placement.

Assisting with discharge planning and transition support activities; collaborate with OCES staff, SNF discharge planners, and appropriate community resources.

Completing nursing assessments as needed to establish clinical eligibility for home care programs and services to help transition the consumer home safely with the support they need to remain at home.
What you'll bring:

Current Massachusetts RN License: degree preferred.

Two years' clinical/community-based experience.

Extensive knowledge of the medical, emotional, and social needs of elders and people living with disabilities.

Great assessment and communication skills needed.

Strong ability to work independently.

Proficient with Microsoft Office Suite or related software.
