Essential Job Functions:
The LVN Discharge planner is responsible for providing case coordination and a full range of social work services to all levels of care including assessment, treatment planning, therapeutic interventions, and transitional planning in an interdisciplinary team environment consistent with the position’s qualifications, profession practices and ethical standards. Demonstrates accountability for and contribution to program development, quality improvement, problem solving and customer satisfaction. Must be willing to work in Boerne at our Menger Springs Campus and the San Antonio Campus, the Meadows.
- Helps guests and families cope with the crises of life-threatening illness, traumatic losses, acute and chronic diseases and disabilities.
- Helps prepare guests and families to successfully transition to the next level of care.
- Organizes and leads social group work activities focusing on needs of patients and family members.
- Participates in the initial and ongoing assessment of the patient, placing special emphasis on psychosocial aspects and formulates a psychosocial treatment plan as deemed appropriate for the overall treatment planning.
- Develops and implements social work programs which consist of individual, group and family interventions.
- Provides professional expertise in cooperation with other health professionals in the admission, evaluation, treatment and follow-up of care and services.
- Completes all documentation as mandated by policy and regulatory agencies within specified time limits, plus documents the course of patient care, including progress made and continuing need for treatment.
- Formulates transitional planning, specifying the psychosocial treatment and interventions required and how they will be provided to include coordinating external services such as ordering of medical equipment to ensure a smooth transition.
- Utilizes satisfaction surveys to collect guest/customer feedback on their experience with their stay and coordinates with the interdisciplinary team to enhance the customer experience.
- Participating member of quality assessment and performance improvement committee (QAPI).
- Able to assist in providing social services to long term residents as assign.
- Other duties as assigned.
Qualifications:
Education: LVN License Valid in Texas. Bilingual preferred.
Training and Experience: Experience working with the elderly in a healthcare setting such as short-stay, acute care, rehabilitation, and/or long-term care facility is preferred.
Certifications/Licenses: Licensed by the Texas State Board of Nursing.
Job Knowledge: Knowledge of basic social work skills and interventions. Demonstrates professional skills, interventions and an understanding of interdisciplinary team approaches. Experience making a clear assessment and develops appropriate treatment plans; organizes and carries out workload based upon needs and priorities; skills in problem solving to remedy adjustment concerns or conflicts; displays sound knowledge base in clinical interventions with individuals and groups.
