The Opportunity
As we expand and transform the primary care experience, we are looking for a virtual Registered Dietitian and Certified Diabetes Care and Education Specialist to join a proactive, multidisciplinary, technology and data focused team directed toward innovating, developing and improving programs. This includes human centered pathways, protocols, processes, metrics, materials, policies to support and educate medicaid patients (18 to 64 years of age) living with uncontrolled Type 1 and Type 2 diabetes. This role will be part of a team and support complex members with uncontrolled diabetes and high social and behavioral needs. This role will have a direct impact on many patient’s lives in a meaningful way.

In collaboration with the Vayu Health clinical team, you will boost the level of support available, build therapeutic relationships, maintain cultural competency, and assist in serving a group of patients through building and maintaining trusting relationships.
Specific Responsibilities
Care for a patient panel of 50-100 patients living with diabetes and potentially other chronic conditions, with combined remote and primary care-based team members
Serve as a consistent team member for the patient panel, which entails serving as primary point of contact for patients and facilitating connections within the community and other team members
Utilize teaching techniques that are sensitive to the cultural and learning preferences of the person with diabetes to: teach diabetes self-management skills, coach on glucose testing, support continuous glucose monitoring (CGM), perform general medication titration and dosing by protocol, deliver diabetes specific training (i.e. carb counting, hypoglycemia, diabetes ketoacidosis), partner with provider on insulin delivery, and other critical skills
Complete comprehensive assessments for each patient to develop an individualized care plan based on the patient identified needs and goals
Collect data and report metrics related to the Diabetes Program
Participate in design thinking, problem-solving, creating and implementing new process and procedures
Actively participate in virtual daily team huddles and largely virtual weekly case conferences to collaborate, advocate, and confer on patient health needs to develop and improve person-centered action plans
Actively participate as a subject matter expert and collaborator of the Diabetes Program
Engage in target setting for the new clinical initiatives and managing those targets, including decreasing unnecessary hospitalizations, emergency room visits, and unnecessary specialty referrals and participate in the quality improvement process to effect change.
Apply systems practice and population health management to support achievement of the Quadruple Aim (reduced costs, better outcomes, improved experience, and improved work life for healthcare providers)
Demonstrate organization and flexibility in patient care assignments to adequately perform the job functions and satisfy the needs of the patients
Act as a mentor and trainer for other members of the team as assigned
Perform other duties as assigned
Education and Licenses Required:
Qualifications:
Required:
Bachelor’s or Master's degree in Nutrition, Dietetics, or a related field.
Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential.
Certified Diabetes Care and Education Specialist National Certification required.
Minimum of 3 years of experience working with members with Type 1 and Type 2 diabetes.
Excellent communication and interpersonal skills.
Proficient in using telehealth technologies and electronic health records.
Strong organizational skills and the ability to manage a remote caseload effectively.
Knowledge of current diabetes management practices and nutritional guidelines.
Experience partnering with providers on starting insulin, the monitoring required for dose adjustments, safety needs per protocol
Experience with continuous glucose monitoring systems

Must reside in the state of California
Willingness to travel for in-person meetings approximately every three months
Preferred:
Bilingual in English and Spanish preferred.
Experience in virtual/remote healthcare delivery preferred.
Experience working with Diabetes technology and devices
Experience with remote monitoring
Experience with leading Diabetes clinical training and education
