ACCOUNTABLE FOR:
· The Certified Nursing Assistant (C.N.A.) performs tasks that involve providing specific personal services for the residents, which are supportive and complimentary to nursing practice.
· All tasks will be performed in a way that they will attain and maintain the highest practicable physical, mental and psychosocial function of the resident while maintaining the resident’s sense of dignity and privacy.

requirements:
- Must not have been found guilty of abusing, neglecting or mistreating residents in a health care setting.
- Must have knowledge of regulatory and compliance requirements regarding the LTC/SNF environment.
- Must have full understanding and working knowledge of the C.N.A. license scope of practice
- Must be free from illegal use of drugs and freedom from use of and effects from drug and alcohol use from the work place.
- Must have a working knowledge of personal computer and software applications used in job functions (word processing, graphics, spreadsheets, etc.).
Language Skills:
- Must remember that English is the language of choice in all of New Vista Facilities and adhere to that practice.
- Ability to provide narrative documentation in the resident’s medical record in English. Have the ability to present information professionally and respond to questions from Leadership, Individuals/Employees, and the public.
Education and/or Experience:
· Must have C.N.A. experience in a LTC setting for 3-5 years (preferred)
· Prefer to have Subacute and Rehabilitation nursing experience for at least 3 year
· Must have at least a 10th grade education. Prefer GED or HS Diploma
· Have a thorough knowledge of the nursing process/assessment and license scope of practice.
· Be able to demonstrate the ability to provide quality care with high acuity residents.
· Communicates and works effectively with people
· Must Demonstrate initiative and utilize time efficiently
· Ability to speak, read and write English clearly and legibly.
· Maintains current CPR certification
· Is courteous and polite at all times.
· Complete and pass a clinical skills competency skills test during the first 90 days of employment.
· Have evidence and information about the course of study completed for certification.
certiFICATES/LICENSES
· Have current Certified Nursing Assistant license from the State of California.
· Must maintain current CPR certification
· Must maintain current fire safety certification.
PHYSICAL REQUIREMENTS:
· Able to maintain emotional Stability
· Must be in good physical condition to meet the demands of an active position to include extensive walking, moderate sitting, standing and lifting
· Physically able to perform CPR
· Will be required to lift and/or transfer residents, medical equipment and supplies etc.
· Will pass a pre-employment and yearly physical exam there after in addition to T.B. screening tests.
ESSENTIAL FUNCTIONS
RESIDENT CARE
· Will possess a genuine interest and concern for geriatric and disabled residents
· Will maintain a pleasant, courteous manner with residents, families and staff at all times. Customer Service is how we get and keep our residents.
· Completes work assignments on a timely basis.
· Acceptable personal grooming and cleanliness, in accordance with C N A uniform policy.
· Provide a suitable safe environment for residents by giving attention to cleanliness, ventilation, lighting and furniture arrangements.
· Ensures that all rooms are resident’s bathrooms are organized.
· Prepares room for receiving new admissions.
· Assists residents to ensure their cleanliness, grooming, nourishment, rest, activity and elimination are provided in a manner conductive to the resident’s comfort, safety and provides dignity and privacy at all times.
· Preserves the dignity and self-respect of all residents at all times. Reports any residents concerns on a timely basis to charge nurse and nursing shift supervisor.
· Practices confidentiality about the resident’s and nursing care activities. Knocks before entering each resident’s room. Introduces self to resident. Wears name tag.
· Ambulate assigned residents.
· Performs ROM on assigned residents as appropriate and safe.
· Keeps alert to the resident’s condition and reports changes in resident’s attitude, appearance, behavior, and physical ability to the charge nurse.
· Performs selected nursing procedures which do not require professional evaluation as assigned and instructed by the charge nurse with in the C.N.A. scope of practice.
· Ensures that each resident receives excellent skin care including perineal care utilizing peri wash.
· Utilizes good hand washing techniques when performing nursing care.
· Practices good infection control measures.
· Practices conservation of equipment and supplies, keeping them clean and ready for use. Reports any equipment and supply needs to the charge nurse.
· Practices first aid measures as applicable and as instructed by the charge nurse,
· Promptly answers all call signals, whether to assigned residents or others. Call lights must be answered within 3 minutes.
· Transports residents as needed throughout the facility.
· Assists residents in their recreational and social activities.
· Recognizes and respects the individual emotional, social and cultural needs of residents, including their religious beliefs.
· Participates in care plan process by attending resident’s care plan conference as requested.
· Weighs residents as instructed.
· Assists with admission, transfers and discharges.
· Keeps track of resident’s input and output and bowel elimination as ordered by the resident’s physician.
· Participates and is a vital part of the Bladder and Bowel Retraining Program on assigned residents participating in the program.
· Is knowledgeable of Nursing Policy and Procedures as related to the Certified Nursing Assistants Function.
· Participates in Resident Care Conferences as directed, contributing knowledge about resident’s daily ADL’s and required daily care.
· Keeps written notes of specific assignments and records pertinent information about the resident including daily ADL’s, weekly summaries, change of condition, B & B Training, BM record, I & O, and meal percentages. Documents accurately on daily flow sheet.
· Cleans bedside tables and closets and monitors both, for appropriate labeling of personal supplies.
· Attends 24 hours of in-service annually, orientation and mandatory classes.
· Maintains Certification as mandated by State and Federal Regulations and ensures that his/her employee file has updated information.
MEDICARE/SUBACUTE UNIT
· Meet daily with Medicare/SubAcute Charge Nurse to discuss change of conditions, resident concerns, and other clinical concerns.
· Set up oxygen equipment as required by the charge nurse
· Participates in daily care plan revisions as requested by the charge nurse.
· Works collaboratively with the restorative nursing assistant to maintain and improve resident functional levels.
Customer Service
- Must recognize that the resident/family is the #1 concern and take utmost care to provide service and resolve any concerns they may have in a timely manner.
- Must recognize that the 1st care planning meeting is the most crucial in ensuring that customer service needs are being met.
- Must recognize that our most important asset is our employees and they must be recognized and valued.
· Creates and maintains an atmosphere of warmth, personal interest and positive emphasis as well as a calm environment through-out the facility.
· Assists the staff in understanding their duties and improving every facet of their performance.

- Must have the ability to define and solve problems collects data, establish facts and draw valid conclusions. Is involved with Residents, Family Members, Personnel, Visitors, Government Agencies/Personnel, etc., under all conditions/circumstances.
- Understands that at times residents, families and other staff may present a very negative attitude towards them.
- Communicates with the medical staff, nursing personnel and other department supervisors and staff.
- Maintains a Partnership with the Residents, their families, support personnel, etc., to assure that the residents’ needs are continually met.
Leadership
· Sets a good example for all staff of what a C.N.A should be.
Communication
· Communicates any and all changes of condition to the charge nurse and nursing supervisor.
· Reports to the Abuse Coordinator any suspected abuse.
· Reports safety concerns to the Charge Nurse immediately.
Financial Stewardship Responsibilities
· Utilizes resident care supplies with economy.
CONFIDENTIALITY
· Handles all resident communication and documentation in a confidential manner as per HIPPA guidelines.
· Does not release resident information / medical records without following facility policy.
SAFETY
· Provides a safe environment by maintaining personal, physical health and mental alertness.
· Completes yearly health evaluation on a timely basis
· Attends required safety programs.
· Adheres to stated dress code.
· Notifies appropriate person of any possible safety concerns.
· Utilize equipment properly and safely.
Workers Compensation Functions:
· Report all new workers compensation incidents to the DSD immediately.
Working Conditions/Physical and Sensory Requirements (With our without the aid of Mechanical Devices):
- Works in office areas as well as throughout the facility and its’ premises.
- Sits, Stands, Bends and lifts and moves intermittently during working hours.
- Is subject to frequent interruptions.
- Is involved with Residents, Family Members, Personnel, Visitors, Government Agencies/Personnel, etc., under all conditions/circumstances.
- May be subject to hostile and emotionally upset Residents, Family members, personnel, visitors, etc.
- Works beyond normal working hours and on weekends and holidays when necessary.
- Is involved in community/civic health matters/projects.
- Attends and participates in continuing education programs.
- May be subject to falls, burns from equipment, odors, etc. throughout the work day.
- May be exposed to infectious waste, diseases, conditions, etc., including exposure to the AIDS and HEP B viruses..
- Must possess sight/hearing senses or use prosthetics that will enable theses senses to function adequately so that the requirements of the position can be fully met.
- Must be in good health and demonstrate emotional stability.
- Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
- Must be able to assist in the evacuation of Residents.
