We are seeking a compassionate and experienced Case Manager to join our client’s team, dedicated to supporting older adults/geriatrics in maintaining independent living. The ideal candidate will possess a Bachelor's degree in a related field, along with previous experience working with older adults and a strong understanding of the challenges they may face. This role involves conducting assessments, developing care plans, coordinating services, and advocating for the needs of older adults to promote their well-being and quality of life.
Responsibilities:
- Assessment and Care Planning: Conduct comprehensive assessments of older adults' physical, cognitive, emotional, and social needs to identify strengths, challenges, and goals. Collaborate with clients and their families to develop individualized care plans focused on supporting independent living and enhancing overall quality of life.
- Service Coordination: Coordinate a range of services and resources to meet the needs of older adults, including healthcare, transportation, home care, meal delivery, social activities, and financial assistance. Advocate for clients to ensure they have access to necessary supports and services, navigating systems and overcoming barriers as needed.
- Monitoring and Support: Provide ongoing monitoring and support to older adults to ensure their needs are being met and their goals are being addressed. Conduct regular check-ins and reassessments to evaluate progress, identify emerging needs or concerns, and adjust care plans as necessary to promote stability and well-being.
- Crisis Intervention and Problem-Solving: Respond to crises and emergencies involving older adults, providing immediate support, assistance, and intervention as needed. Employ problem-solving skills and crisis management techniques to address challenges and mitigate risks, working collaboratively with clients and other stakeholders to achieve positive outcomes.
- Education and Advocacy: Educate older adults and their families about available resources, support services, and entitlements relevant to aging and independent living. Advocate for the rights and preferences of older adults, ensuring their voices are heard and their choices respected in decision-making processes related to their care and living arrangements.
Requirements:
- Bachelor's degree in Social Work or a related field.
- Previous experience working with older adults/geriatrics in a social services, healthcare, or community-based setting.
- Must speak Spanish.
- Strong knowledge of aging-related issues, including physical health, cognitive functioning, mental health, and social support needs.
- Excellent assessment, care planning, and case management skills.
- Effective communication and interpersonal skills, with the ability to establish rapport and build trust with older adults and their families.
- Ability to work independently and as part of a multidisciplinary team.
- Compassion, empathy, and a commitment to promoting the dignity, autonomy, and well-being of older adults.
Please note- The annual base salary range is $45k to $52k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
