The Intake Registered Nurse (RN) Coordinator collects, reviews and confirm patient referral information and helps to determine initial eligibility for admission to HQ Home Care program per agency, regulations, and accrediting body standards. Builds and maintains a positive relationship with customers and provides general intake support. Promotes and fosters ongoing collaborative communication with internal/external referrers to establish coordinated home care services. Works under general supervision.

Responsibilities:
- Primary responsibility for reviewing patient referral information to determine initial appropriateness for scheduling of admission to the home care program per agency/regulatory standards.
- Establishes a prompt positive customer service oriented communication pattern with all referral sources and continuously focuses on meeting the needs of the referral source to facilitate patient care transition.
- Obtains patient insurance and payment information from various sources, and submits for home care authorization.
- Distributes referral information to intake team and/or supervisory staff on a timely basis to ensure case assignment and continuity of care.
- Provides home care assessments screening and evaluating to determine appropriate home care services for the patients identified by hospitals and community.
- Determines customer needs and provides accurate and responsive intake to ensure customer satisfaction and best practice outcomes.
- Continuously educates all internal and external customers on the full range of home services that can be provided by HQHC.License, Registration, or Certification Requirements

- Bachelor’s degree in nursing preferred.
- License and current registration to practice as a Registered Nurse in New York State required.
- CPR Certification preferred.

Pay: $32.64 - $65.14 per hour
