**Job Description:**
As a Pediatric Case Manager, you will play a crucial role in coordinating care for pediatric patients receiving Private Duty Nursing services. You will collaborate closely with healthcare professionals, families, and caregivers to ensure the delivery of comprehensive and personalized care plans.
**Responsibilities:**
- Conduct initial assessments and ongoing evaluations of patient needs.
- Develop and implement individualized care plans in collaboration with healthcare team members.
- Coordinate and schedule nursing visits to ensure continuity of care.
- Monitor patient progress and outcomes, making adjustments to the care plan as needed.
- Serve as a liaison between healthcare providers, patients, and families to facilitate communication and support.
- Provide education and support to families regarding care procedures and resources.
- Maintain accurate and timely documentation of patient care activities and communications.
**Requirements:**
- Registered Nurse (RN) with at least 2 years of experience in Pediatric Private Duty Nursing (PDN) or home healthcare.
- Strong clinical assessment and care coordination skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in English; Spanish speaking skills preferred but not required.
- Compassionate and patient-centered approach to care.

Pay: $65,000.00 - $70,000.00 per year
