MDS Coordinator/RN – Responsibilities:
- Coordinate and ensure timely completion of the RAI/Care Plan process according to federal guidelines
- Provide all disciplines a list of those residents whose MDS/Care Plan is due prior to the due date according to Center protocol
- Conduct Care Plan meetings with responsible staff members, interdisciplinary team members, residents, and family members, where applicable
- Coordinate a Comprehensive Assessment (including MDS, CAAs, Triggers narrative CAA summary, Care Plan) on admission, within one year of the most recent assessment, or whenever there is “significant change” in resident’s status. Coordinate quarterly assessments as required
- Where applicable, transmit assessment data to state agencies, monitor, and correct Medicaid rosters and coordinate assessments to maximize Medicaid Case Mix reimbursement
- Review medical records as directed by the DON to ensure proper documentation is maintained relating to residents’ functional and clinical statuses
- Sign off on completion of MDS
- Maintain a working knowledge of and ensure compliance with all governmental regulations related to the RAI/Care Plan process and where applicable with the Case Mix reimbursement program
- Educate each discipline on the RAI process and required documentation to ensure accuracy of the RAI
- Maintain professional competence as well as current licenses through participation in continuing education programs, seminars, and training programs.
MDS Coordinator/RN – Qualifications:
- hold a current and valid Registered Nurse (RN) license in the state employed
- Minimum of (1) years’ experience completing MDS’s (3yrs nursing experience preferred)
- Completion of post-secondary education
- Ability to travel to multiple centers for day trips with the occasional overnight stay.
- Must meet all local health regulations and pass post-employment exam, if required
