2. DEA
3. Wound Care Certified
2. 2+ year of experience working for Family Practice with geriatric specialty is preferred, recent graduates welcome to apply.
- Knowledge of CPT, ICD-9, ICD-10 and HCPC codes
- Ability to work effectively in a dynamic, fast-paced family practice.
- Excellent ability to always work and communicate in a professional manner.
Job Responsibilities:
- Perform comprehensive physical examinations of patients of all ages in the comfort of their home.
- Assessment of acute, episodic, or chronic illnesses and ability to report and record for History and Physical reports.
- Practices in accordance with the respective state laws and regulations governing the practice of Registered Nurse in Nevada.
- Understands and complies with long-term care policies and procedures, including those that regulate nursing facilities and assisted living communities.
- Establishes and maintains effective relationships with members and their legal representatives, facility staff, PCPs, and other collaborating providers and nursing staff.
- Performs an initial comprehensive assessment on all newly enrolled members.
- Completes follow-up and post-hospital assessments according to documented standard operating procedure.
- Reports abnormal labs and diagnostics results promptly to supervisor to avoid unnecessary transfers/acute admissions.
- Educates members and/or their legal representatives in disease processes affecting members and ways to manage them effectively, as well as to promote wellness.
- Customer Service - Manages difficult or emotional customer situations; responds promptly to customer needs; solicits customer feedback to improve service; responds to requests for service and assistance; meets commitments.
- Work within a collaborative team of Providers, nurses, and medical assistants
- Communicate with collaborating physician or specialist regarding patient care.
- Perform additional duties as assigned.

Pay: $100,000.00 - $134,762.00 per year
