Looking for outstanding, critical-thinking Licensed Practical Nurses (LPNs) for our Atlanta center for our part-time, every-other weekend 7am-7pm shift . The Fountainview Center for Alzheimer's Disease is privately-owned, has a family-like team of professionals, and has an outstanding reputation in the healthcare sector. The right candidates are expected to:
- Make walking rounds with the nurse leaving the shift or starting the shift, taking or giving report relating to resident conditions and staff circumstances.
- Count narcotics with the nurse starting or leaving the shift.
- Attend daily stand up meeting and go over changes in resident condition.
- Administer medications as ordered by the physician.
- Administer treatments as ordered by the physician.
- Assesses the resident's condition for changes and accuracy of the plan of care.
- Communicate to the attending physician regarding changes in resident's condition and/or results of diagnostic studies.
- Take telephone orders from the physicians and or FNP.
- Carry out orders from the attending physician and/or NP.
- Note the orders and accurately transfer them to the MAR or TAR including upcoming month’s MAR or TAR when deemed necessary.
- Communicate with families regarding condition changes and document such actions in the medical record.
- Document resident's condition and/or changes in the medical record and communicate these changes to all interested parties including managers, physicians and families.
- Monitor/supervise monthly and weekly weights.
- Enter weights in the electronic health record system and communicate changes to the dietary consultant and attending physician. Help develop and carry out interventions to address weight changes.
- Review reports from the electronic health record system to determine if CNA and nursing documentation are completed in a timely fashion.
- Make rounds with the Wound Care Specialist physician and/or Attending Physician as needed.
- Provide direct care to residents as needed, which may necessitate bending, reaching, lifting, pushing, and pulling.
- Ensure proper positioning of residents either directly or with assisting other staff, which may necessitate bending, reaching, lifting, pushing and pulling.
- Assist with feeding residents and overall monitoring resident dining rooms to ensure all residents are fed timely, properly, dignity is maintained, and intake records are accurate.
- Maintain resident charts, thin charts using facility policy and procedure for thinning, do 24 hour chart checks to ensure orders are noted and carried out, diagnostic studies are conducted and placed in the medical record.
- Assess and conduct treatments to residents as ordered by the physician, which may necessitate bending, lifting, reaching.
- Conduct skin assessments on admission, on return from outside appointments, or hospital visits, with showers/baths, and as needed to ensure skin integrity and interventions are needed. This may necessitate bending, lifting, reaching, pushing, pulling and leaning over.
- Document resident behaviors in the electronic health record system to assist in determining if behavior management plans of care are appropriate. Work with the DON/ADON, physician, or NP to address behavioral changes as documented in the electronic health record system.
- Carry out pharmacy and dietary consultant recommendations as approved by the attending physician/NP.
- Complete documentation such as skin assessments and monthly summaries per assignment.
- Maintain cleanliness and neatness of medication carts, treatment carts, and refrigerators within acceptable temperatures, which may require bending over, lifting objects.
- Obtain supplies/equipment so that the interdisciplinary team members can provide care to residents.
- Enter physician and/or NP orders in the electronic health record system.
- Ensure that equipment in is in good working condition or ensure that non-working equipment is taken out of operation to meet facility safety standards.
- Review crash carts to ensure emergency procedures are carried out smoothly.
- Complete risk assessments per facility policy. Put interventions in place to address identified risks.
- Assess residents, make changes in the plan of care to help prevent and/or address changes in resident condition.
- Oversee activity programs to ensure that the philosophy and mission of The Fountainview Center are met and the recreational and emotional needs of the residents are met.
- Start IVs, maintain IVs, administer fluids, and/or medications via venous access.
- Administer oxygen and or measure oxygen saturation levels as ordered by the attending physician or by using nursing judgment, which may require pushing, pulling or moving heavy oxygen concentrators.
- Supervise, counsel, and discipline staff for violations of facility policies, procedures or regulations.
- Attend routine nursing meetings to discuss resident needs and interventions to address these needs when necessary, in collaboration with the DON/ADON.
