An established long-term care organization is currently seeking Nurse Practitioner Supervisors for their Brooklyn, Bronx and Queen locations. The NPS will team up with community primary care doctors to oversee participant care via the Interdisciplinary Team. They'll work on coordinating functional, clinical, and psychosocial needs with an independence and wellness focus. This role acts as a bridge between the participant, community physician, and plan staff for care planning, goal development, condition updates, and preventive services.
The base pay range for this position is $80 to $83 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer. Responsibilities:
- Responsible for overseeing and directing Nurse Practitioners in their assigned location, and aiding in the supervision of other clinical staff at the Diagnostic Treatment Center as necessary.
- Conducts evaluations and yearly performance reviews for direct reports, collaborating with other DTC Managers to deliver annual performance feedback to clinical staff at the DTC as required.
- If the Medical Director is absent or as directed, the NPS will act as the representative or assume the role of the Medical Director at their location.
- As a minimum requirement, the NPS will conduct annual wellness assessments for all participants at the assigned site, ensuring a comprehensive understanding of the intricacies and multiple underlying health conditions of frail and elderly individuals. For participants at higher risk levels, these assessments will occur more frequently to effectively address their specific needs, as outlined below.
- Offers urgent care services to participants and facilitates immediate access to care.
- Assesses and addresses participants with sudden changes in health condition in a prompt manner.
Requirements:
- Masters Degree in Nursing (NP) or Doctor of Nursing Practice (DNP)
- Certified Nurse Practitioner license
- NY State Nurse Practitioner License required
- At least two years in clinical nursing practice in-home care, care management, or related field is required.
- DEA Certification is required.
- AANP or ANCC Board Certification is required.
