Job Summary
Provides prescribed medical treatment and personal care services to our clients and employees. Under the direction of an RN.
Essential Job Duties
1. Administers prescribed medications and treatments in accordance with approved nursing techniques.
2. Works in cooperation with visiting professionals, i.e., therapy, and psychiatrist.
3. Observes patient, records significant conditions and reactions, and notifies supervising RN and/or physician of client condition and reaction to drugs, treatments and significant incidents.
4. Orders medications from pharmacy.
5. Takes temperature, pulse, blood pressure and other vital signs to detect deviations from normal and assess condition of client.
6. Maintains all documentation in accordance with licensing standards.
7. Utilizes the electronic data collection system to enter all applicable documentation.
8. Checks for follow-up appointments to ensure scheduling.
9. Review of MARs for completion and accurate documentation.
10. Is available after hours in rotation with other medical staff for health changes and other client-related medical situations as directed by the RN.
11. Attends staffings to assess and review client’s care and needs.
12. Participates in department or unit quality improvement activities.
13. Conducts training as directed by the RN.
14. Applies first aid when needed.
15. Inventories and requisitions supply.
16. Other duties as assigned.
- Cycle fill medications – will be delivered to each group home and checked in by reviewing the medication at the home. LVN to look for prescription labels matching the MARS and orders in TMP. LVN to remove medications that have expired or have been discontinued.
- LVN to consult with assigned RN when extra medications appear to be left from the prior month.
- LVN to follow the procedure for medication counts.
- The LVN will document in TMP the following to include but not limited to: speaking with the pharmacy; physician’s offices; families/LARs; staff; any tasks delegated by the RN
- LVN will communicate with the on-call RN when necessary to relay information that may be related to a condition that is not stable; communicate with the assigned RN when identifying issues from “on-call Report” and/or IBAMs that seem to be a trend; communicate with the assigned RN anytime you are uncertain or question issues related to client care.
- The LVN will review all Physician Progress Reports and document the appointment and results on a case note.
- The LVN will create a system to track consolidated orders.
- The LVN will review individual medical charts so that all charts are reviewed quarterly
- Other duties may be assigned.
