Job Overview:
We are seeking a CA Registered Dietitian to join our healthcare team. The Dietitian will be responsible for the development of dietary/nutritional needs of PACE outpatient clinic participants and to provide nutritional counseling and education to participants and family members/caregivers.
Responsibilities:
- Conduct diagnostic evaluations to assess patients' nutritional status and needs
- Develop personalized dietary plans based on patients' health needs and preferences
- Provide health coaching and education to patients on proper nutrition and healthy eating habits
- Monitor and evaluate patients' progress towards their dietary goals
- Collaborate with internal and external healthcare professionals to ensure comprehensive patient care
- Specialize in areas such as tube feeding, alternative , or sports nutrition as needed
Requirements:
- Bachelor's Degree from an ADA accredited institution with a major in nutrition. Master’s Degree preferred.
- Valid California state licensure as a Registered Dietitian
- Strong knowledge of physiology and its application to nutrition
- Experience in patient assessment, dietary planning, and nutritional counseling
- Knowledge base of health care problems of the frail elderly
- Skilled in physical assessment and triaging.
- Proficiency in utilizing medical nutrition therapy for various health conditions
- Excellent communication and interpersonal skills
- Knowledge of common safety hazards and precautions to establish a safe work environment.
- Skilled in preparing and maintaining records and writing reports and responding to correspondence.
- Skilled in developing and maintaining department quality assurance.
- Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
- Ability to react calmly and effectively in emergency situations.
