Where do you fit?
To be a successful Crossover Permanency Specialist you must become able to do the following always:
- Ensure a trauma informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery.

Work with youth who move between the juvenile justice and child welfare systems utilizing the Crossover Youth Practice Model (CYPM) to address the unique needs of the youth.

Develop partnering relationships with the parents and youth, with the goal to work toward reunification. To help achieve reunification as soon as safely possible, permanency and specialized case management, engage the family in a meaningful relationship and to utilize assessment and case planning to guide services.

Support youth and families by collaborating with a variety of specialized service areas within the program. Assist them in connecting with specific community resources and specialized services to benefit the family.

Ensure documentation is completed and/or upload timely according to company policy and procedures.

Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.

Back up on-call to the on-call department.

To qualify for this job, you need to have:

Bachelor’s degree in social work or related field preferred.

Prior work experience in child welfare services preferred.

Ability to travel, driving company or personal vehicle including at night.

21 years of age. Must pass a background check, fingerprinting, and physical and drug screening.

Maintain company-required training.

Ability to work flexible schedule, including some evenings and weekends. Employee subject to outside environmental conditions occasionally, inside conditions frequently, with protection from weather conditions but not necessarily from temperature change.
