A Direct Support Professional Lead provides individually focused care and support to ensure each person can make choices and meet outcomes. You will be encouraging growth toward independence in activities of daily living, socialization, community involvement, and support in expression of choices.
Accountabilities of a Direct Support Professional Lead

Provides care and support to all individuals to assure that they are safe and all needs are met in accordance with the ISP and or Behavior plans

Coaches and mentors the Direct Support Professional team to ensure quality services are being rendered.

Supports individuals to develop goals, make informed choices and follow-through on responsibilities.

Participates in assessments to determine the chosen outcomes, needs, preferences and capabilities of the individual by gathering information and informing the individual about what to expect throughout the assessment process

Advocates for the individual and/or promotes self-advocacy to meet individual’s needs by assisting in the removal of barriers and/or by sharing views with team members to create a solution to advocacy concerns.

Communicates positively and effectively with all internal and external team members on issues, problems, successes of each individual to support achievement of outcomes and assure quality service.

Mentors and leads Direct Support Professionals by example, to provide the highest quality, person-centered support to individuals.

Provide detailed, accurate and timely documentation for all services and supports provided.

Participates in assessments and mandated trainings to support the needs of the program and individuals being supported.
Expectations of a Direct Support Professional Lead

Provide feedback for individual’s Monthly and Quarterly Reports
-
Maintain well organized files for each individual, file documents and reports
-
Assists in Support Coordination monitoring meetings
-
Coordinate and provide meaningful weekly activity schedules of the individuals and home
-
Coordinate monthly fire drills and monthly site checklists along with any another monthly tasks
-
Train OJT pre-service staff to ensure to needs of the residents and the home are explained
-
Other duties as needed and/or requested

The tools you need

Associate’s Degree or 60 credits
-
Must be at least 18 years of age;
-
Must have a valid PA/NJ/DE Driver’s License and 1 year of driving experience
-
Must be available to attend the 3-week paid hourly training
