Responsibilities:
- Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.
- Assist with activities of daily living, such as bathing, dressing, and feeding.
- Collaborate with healthcare team members to develop and implement patient care plans.
- Document patient information, including medical history, symptoms, and treatment progress.
- Educate patients and their families on health conditions, medications, and self-care techniques.
- Utilize electronic medical record systems (such as Athenahealth) to maintain accurate and up-to-date patient records.
- Follow aseptic techniques to prevent infection transmission.
- Provide health coaching and support to patients for managing chronic conditions.
- Collaborate with case managers to coordinate patient care and ensure continuity of services.
- Administer medications and treatments as prescribed by healthcare providers.
- Perform catheterization and dialysis procedures as needed.
Requirements:
- Valid Licensed Vocational Nurse (LVN) license in the state of [state].
- Minimum [number] years of experience working as an LVN in a healthcare setting.
- Proficiency in toddler care and understanding of their unique needs.
- Knowledge of medication administration procedures and dosage calculations.
- Familiarity with electronic medical record systems (such as Athenahealth) for documentation purposes.
- Ability to perform aseptic techniques to prevent infection transmission.
- Strong communication skills to effectively interact with patients, families, and healthcare team members.
- Experience in case management and coordinating patient care is preferred.
- Ability to work independently and prioritize tasks in a fast-paced environment.
