The Hurtt Family Health Clinic is looking for a full-time on-site Grant Writer and Manager of Community & Patient Services to work within a care team model to oversee Hurtt’s Community and Patient Services Department, including programs that support Hurtt’s health services through community and patient engagement, outreach, enrollment to public benefits, education, patient navigation, community linkages, and social determinants of health services. This will include working with community partners and local and state organizations.
This position requires a highly organized individual with initiative to pursue program improvements and emerging community health initiative to best serve our patients and community. The goal is to improve access, quality, health outcomes, and patient satisfaction by addressing the social drivers of health care. In addition, work with CEO to research funding sources for federal, non-federal and private grant opportunities for Federally Qualified Health Center; writes, submits grant proposals and project manage awarded grants while updating and maintaining the grants database.

The Hurtt Family Health Clinic is a local community health clinic organization that provides medical, dental, vision, chiropractic, acupuncture, and behavioral health services to low-income patients in Orange County, California. Our organization has four clinic locations within Orange County located in Tustin, Santa Ana, and Anaheim.
Requirements
- Master in Public Health degree preferred. Master’s degree in similar fields preferred but not required. Bachelor’s degree in communications, business administration, grant administration, or similar field required.
- Understanding of private, federal, and county-funded non-profit funding endeavors and strategies
- Demonstrated knowledge of diverse grant application procedures and best practices in the nonprofit sector.
- Demonstrated knowledge of social service program development and implementation
- Comprehensive experience in researching and identifying funding opportunities, and analyzing diverse data and possessing the ability to work collaboratively and independently to achieve stated goals
- Must be able to work beyond normal working hours, including weekends

Pay: $75,000.00 - $85,000.00 per year
