QUALIFICATIONS:
- Masters prepared in one of the recognized mental health professions.
- Colorado State professional license in good standing, LCSW or LPC preferred.
- 2+ years’ Experience working in integrated care and/or clinical setting preferred.
- Experience providing brief behavioral interventions to patients with chronic conditions preferred.
- Bi-Lingual (Spanish/English) preferred.
- Experience working with diverse populations including immigrants/refugees as well as clients with diverse socioeconomic backgrounds and levels of education.
BASIC FUNCTION: The Integrated Behavioral Health Provider (IBHP) operates closely with the Primary Care team to offer behavioral health consultation and short-term outpatient counseling services within the primary care setting. This involves providing brief, targeted interventions across all age groups, including children, adolescents, adults, and families, with the goal of improving patients' psychosocial functioning during challenging circumstances.
RESPONSIBILITIES:
- Collaborates with Primary Care Providers, the IBHP assesses, implements, and educates patients on self-management strategies to address various issues such as grief, caregiver stress, substance abuse, new diagnoses, and other behavioral health conditions.
- Delivers brief behavioral health interventions utilizing evidence-based techniques like behavioral activation, problem-solving treatment, and motivational interviewing.
- Conducts short-term focused counseling sessions tailored to individual patient needs. For cases requiring longer-term care or specialized intervention, the IBHP refers patients to other behavioral health providers.
- Using cognitive-behavioral methods, motivational interviewing, and mindfulness-based interventions, the IBHP assesses patients' readiness for change and identifies barriers to care and compliance.
- Collaborates with patients to establish manageable treatment goals and objectives while providing support and education to patient family/caregivers.
- Stays informed about community resources for appropriate referrals and evaluate crisis situations to apply suitable interventions.
- Ensures timely documentation in the Electronic Health Record (EHR) to maintain accurate records of patient care and progress.
- Performs a variety of tasks and other duties as assigned.
