Some of the duties the RN Weekend Nurse will be responsible for are below:
- Performs Admits as needed to ensure patient care and staff utilization
- Triage calls from patients, families, caregivers, or facilities.
- Completes an initial, comprehensive and ongoing comprehensive assessment of the patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
- Provides professional nursing care by utilizing all elements of nursing process.
- Assesses and evaluates patient’s status by:
- Writing and initiating plan of care
- Regularly re-evaluating patient and family/caregiver needs.
- Participating in revising the plan of care as necessary.
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan that established goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan of care.
- Facilitate the patient’s efforts toward optional comfort care.
- Maintains comprehensive working knowledge in the field of managed care and shares information with appropriate organization personnel.
- Perform other duties and activities as delegated by the Patient Care Manager or Administrator.
- On-Call staff will be able to reach a patient within 1 hour. (There may be rare expectations, depending on how far away the patient lives and if staff is with another patient at the time of the page).
- The On-Call Nurse will report weekend patient activities to the RN Case Manager, Patient Care Manager and the Administrator.
- The following is meant to be a guide and is not an exhaustive list, but includes many problems that may require a visit from the On-Call Nurse.
- Death or suspected death
- Unusual, severe or uncontrolled pain,
- Difficulty breathing
- Nausea/vomiting not resolved with current medications
- IV Problems
- Report of patient failing
- No BM for four days or more (if taking nourishment)
- No urine output for eight to twelve hours (if taking fluids and having discomfort)
- Increased anxiety and/or confusion
- Duplicate calls regarding the same problem
- Patient/family/caregivers perceives a problem and requests a visit
- Suspected Bleeding
