SUMMARY
Under the direct supervision of the MIHP Manager and in collaboration with the Director of Behavioral Health Services, the MIHP Nurse provides home-based care coordination and educational services to Medicaid-eligible pregnant women and infants from birth to age one, to promote healthy pregnancies, positive birth outcomes and infant health and development.
ESSENTIAL JOB FUNCTIONS including the following.
Provides home-based, individualized educational programming based on clients and their family members and provide crisis interventions as needed, utilizing multi-media approach appropriate for adult learners.
Conducts seminars, workshops and presentations to professional and community groups on topics related to areas of expertise.
Assists with the development and implementation of patient educational programs, standards, and materials.
Obtains required education credits/hours in order to maintain current State of Michigan Nursing License.
Nurtures and maintains knowledge of and a good working relationship with other community agencies/schools. Represent program by participating in meetings/committees as assigned by MIHP Coordinator.
Completes accurate physical, emotional, social, educational, environmental and developmental assessments of clients and their families on an initial and ongoing
basis.
Develops and implements individualized plan of care based on needs identified in psychosocial assessment. Lead multi-disciplinary team planning process for assigned cases. Works cooperatively with physicians and all team members to develop shared plan of care.
Demonstrates principles of CQI through team participation and performance improvement activities utilizing CQI approach to problem-solving.
Represents client needs to other healthcare professionals, including physicians. Encourages client to become informed consumer of healthcare and supportive services.
Informs clients of available community resources and encourage their appropriate use. Encourages client self-determination.
Maintains legible, up-to-date, accurate, complete medical records according to standards required for program accreditation.
Regular and prompt attendance at work.
Monitors and directs the team member activities toward goals for assigned clients.
Provides professional care, respect, recognition, and support to clients and co-workers. Staff must have the ability to maintain a non-judgmental attitude toward client families whose lifestyles and backgrounds vary from one’s own experience.
Maintains confidentiality and privacy, respecting both client and their medical records according to HIPAA Guidelines at all times including use of electronical devices and cellular phones.
Maintains a courteous, helpful, and conscientious attitude in dealing with co-workers and clients and their families ensuring professionalism and confidentiality at all times.

EDUCATION, CERTIFICATES, LICENSES, AND/OR EXPERIENCE
Must possess a Master of Science in Nursing or a Bachelor of Science in Nursing and at least one year of experience providing community health, pediatric and/or maternal and infant nursing services; or a nursing diploma or Associate’s Degree in Nursing and at least one year of experience providing community health, pediatric or
Maternal/infant nursing services. Hold a current State of Michigan Registered Nurse License and current State of Michigan Driver’s License.
