Responsibilities:
- Review Value Based Care Program performance data for attributed lives by program and create action plans that support population health management and the organizational goals.

Evaluate payer and internal reports to identify provider and office trends and opportunities.

Coordinate and facilitate monthly meetings with providers and staff to review current topics, metrics, and gap performance and opportunities.

Conduct patient outreach in accordance with HIPAA and patient privacy guidelines to support quality-related programs and ensure target goals are achieved.

Work closely with other Population Health Coordinators and Population Health support department to achieve organizational goals.

Responsible for working with patient and a patient’s care team to coordinate change readiness, needs assessment, and develop an individualized treatment care plan.

Collaborate with the patient and a patient’s care team to define and assist in achieving maximum levels of wellness and independence.

Assess barriers and or social determinates of health when a patient has not met treatment goals and is not following plan of care.
Qualifications:
- Graduation certificate from accredited LPN / RN School of Nursing

12 months related experience and/or training
-
LPN or RN active license
-
Prior Family Medicine experience preferred, but not required
