POSITION DESCRIPTION
Responsible as an integral member of the hospice care team reporting to the Volunteer Coordinator to provide volunteer patient sitter and homemaker care and patient errands consistent with Continuum Care’s policies and procedures.
QUALIFICATIONS
RESPONSIBILITIES
Principal responsibility is to provide volunteer hours to CCI’s patients and their families.
Maintain confidentiality at all times.
Provide work availability each week for the following work.
Demonstrates reliability of volunteer responsibilities as integral component of care for patients and their families
Provide home sitter service as a companion.
Provide light housekeeping in order to assist the patient and /or patient’s caregiver.
Provide patient and family errands when able. This can include dropping off and picking up mail, picking up medications, delivering medical supplies and light weight equipment to the patient's home.
Timely documentation and submission of assigned activities and timesheet.
May be included in IDG weekly meetings to provide updates of her/his patient(s).
Errands and other office assignments as requested by the Volunteer Coordinator.
Contacts: Patients, families, volunteers, staff members, hospital, community personnel, and the public.
Physical Demands: Physical exertion to include walking, lifting, pulling, driving, step climbing, flex hours and travel. Prolonged or considerable walking or standing.
Environmental Conditions: Exposed to infections, communicable diseases, odors, blood, excreta and hazardous materials.
Working Conditions: Exposed to terminally ill patients and their families.
