The NHTD/TBI Waiver Service Coordinator is responsible for providing comprehensive case management services to clients, including needs assessment, service plan development, coordination of services, and monitoring of progress towards goals. The Service Coordinator works collaboratively with clients, service providers, and community resources to ensure that clients receive the services and support they need to achieve their goals.
Roles and Responsibilities
- Conduct comprehensive needs assessments with clients to identify their goals, strengths, and barriers to success. Minimum of 1 face-to-face home visit required.
- Develop individualized service plans that are client-centered, strength-based, and goal-oriented.
- Coordinate with service providers and community resources to ensure clients receive the services and support they need.
- Monitor client's progress towards their goals, adjust service plans as needed, and document progress in case files.
- Provide crisis intervention and support to clients as needed.
- Maintain accurate case records and documentation in accordance with agency and regulatory requirements.
- Participate in team meetings, case conferences, and trainings as required.
- Provide referrals to other services and resources as needed.
- Maintain confidentiality and ethical standards in all aspects of job performance.
Ratio of Waiver Participants to Service Coordinator
Full time Service Coordinators for HCBS/TBI waiver participants may not exceed a caseload of twenty five (25) waiver participants.
Service Coordinators providing services to HCBS/TBI waiver participants on less than a full time basis must limit their caseload proportionately. For example, a Service Coordinator working 50 percent may not exceed a caseload of eight (13) HCBS/TBI waiver participants.
Qualifications:
- Master's degree and 1 years of case management experience or
- Bachelor's degree plus 3 years experience or
- Associate's degree plus 5 years experience.
