Deblin Health Concepts & Associates, Inc. (DHC) is a for-profit mental health service agency providing a wide array of services in, San Antonio, TX area, Austin, San Marcos, Houston, Humble, Beaumont, Cleveland, TX service areas. Mental health counseling, intensive case management, skills training, psychosocial rehab, acute day programs, geriatric counseling, in-home therapy, tele-counseling, hospital discharge planning/liaison services are among some of the services provided.
Responsibilities:
The Case Manager is responsible for the implementation of plans in the direction of the Case Manager Supervisor for Intensive Case Management (ICM) to help each client achieve optimal stability and functioning. He/she familiarizes him/herself with priority client cases, executes tasks in a timely manner, develops resources and establishes key contacts in the database for immediate access of resources to serve the needs of the client. Case Manager will implement agency operations, policies, and procedures and will ensure that the lines of communication between Administration, the field (therapists, and family members, community providers) and the client remain open.

Master Social Worker (MSW)

Valid Drivers License

Experience in providing mental health services, preferable and out-patient settings.

Position Summary:
The Case Manager functions as an active part of the treatment team assisting to provide services to those clients with a Level of Care 3 and/or 4. The Case Manager accomplishes client care by assessing treatment needs; developing, monitoring, and evaluating the treatment plans and progress in collaboration with the clients’ therapists which directly impacts the clients’ optimal stability and functionality; facilitating interdisciplinary approaches; all assessments/treatments are provided through visits with the clients that are scheduled based on company policy; and attends meetings and training sessions as scheduled by management.
Education:

Master’s Degree in a field of Mental Health Studies / Behavioral Science
Work experience:
-
Minimum of 1 year experience in providing mental health services, preferably in an outpatient setting.
Knowledge, Skills & Abilities:
-
CPR Certification
-
Demonstrable work experience in case management with clients with higher needs, as indicated by the Level of Care -Level 3 or 4.

Excellent knowledge of case management principles, healthcare management, and reimbursement procedures.

Excellent knowledge and experience with successfully completing documentation (Master Treatment Plan, Coordination of Care notes, etc.).

Previous experience with psychological aspects of care through working in mental health care.

Excellent organizational and time management skills.

Able to effectively work remotely with minimal supervision.

Familiarity with computers and other technical resources.

Problem-solving skills and ability to multi-task.

Job Duties
-
Coordinates and provides care that is safe, timely, effective, efficient, equitable, and client centered, and promotes quality and cost-effective interventions and outcomes. Develops effective working relations and cooperate with Deblin team throughout the entire case management process.

Handles case assignments, drafts Master Treatment Plans, reviews case progress and assist with determining case closure – all in collaboration with the clients’ therapists. Helps clients achieve wellness and autonomy.

Arranges referrals and coordination with social services, health and governmental agencies for the client as needed. Helps the clients make informed decisions by acting as their advocate regarding their clinical status and treatment options. Assesses and addresses client motivational and psychosocial issues.

Interacts with clients to track progress and to ensure client satisfaction. Effectively documents all information, accurately and timely completes all necessary forms, and produces statistical reports within the time frame specified by Deblin policies.

Successfully completes Coordination of Care with the therapists and clients either weekly or biweekly as determined by clients’ level of care.
