Patients, who are otherwise unstable for discharge after an acute care hospitalization, can be transferred to a long term acute care hospital (LTACH) to receive continued nursing care. This continuation of care serves the needs of patients with medically complex conditions, especially those suffering from multi-system complications and needing extended recovery times.

Our Ideal Candidate will:
- Provide care for the Adult or Geriatric patient, ages 18 years and over.
- Assess a patient's condition, learning needs and family dynamics based on systematic data collection utilizing assessment skills and tools appropriate to the respective practice area.
- Develop a comprehensive plan of care and discharge plan based on the nursing assessment and integration of the medical plan utilizing the patient's family and other health care team members.
- Make patient rounds to assess physical and emotional status and to initiate any nursing interventions.
- Responsible for the overall direction and supervision of all patient care during his/her shift.
- Demonstrate the knowledge and skill to carry out the nursing process when providing care to patients.
- Assist with activities of daily living, vital signs, toileting and respond to call lights.
- Documents delivery of health care and nursing process in accordance with specific unit standards and hospital nursing policy.
- Obtains report from Nurse being relieved from duty and provides report to Nurse coming on duty; keeps the clinical team and others, as appropriate, informed about the status of patients and related matters; performs or supervises complete, timely and accurate documentation of patient care.
- Evaluate outcomes of patient care planning and teaching and collaborate with other health care team members to revise the plan according to the changing needs of the patient.
- Communicate effectively and appropriately to patient, family and other health care providers
- Monitor and follow all procedures regarding patient telemetry; notifies Charge Nurse of all telemetry alarms, documents alarms, gives shift reports and reports any malfunctions or equipment failure.
- Know the rationale for the effect of medications and treatments and correctly administer the same.
- Perform the administration and documentation of medications, internal nutrition and treatments per the physician’s order and accurately records all care provided.
- Understand and follow infection control and patient safety protocols.
- Participate in hospital-wide patient safety programs identifying risks to patient safety and reducing healthcare errors.
