Job Title: Registered Nurse Care Manager
Supervisor: Practice Manager
Position Type: Exempt
Salary: Base of $110,000. The pay is negotiable depending on the candidate's work performance.
Incremental Bonus of $15,000 as follows: $1,500 at hire, $2,000 at 90 days, $4,500 at 12 months and $7,000 at 18 months.
This position reports to the Medical Director or designee, the RN will work with diverse populations with multifaceted needs. This position works in collaboration with the physicians, other healthcare providers, and other ambulatory support staff within the organization and with other disciplines to plan and coordinate the delivery of excellent patient care in internal medicine and infectious diseases specialty care practice. This position will provide direct patient care and health education; obtain and document patient medical data; and perform minor, non-invasive tasks and procedures. The RN Care Manager will lead Chronic Care Management, Transitional Care Management, and Medicare Annual Wellness Activities, serving as a care planner, care coordinator, and advocate for the patients of WHI. This individual will be responsible for providing nursing care through the organization, nursing assessments, teaching, nursing care plans, and routine nursing procedures.
SUPERVISION and LEADERSHIP
- Provide nursing leadership in corporate-wide efforts and initiatives.
- Actively supervises clinical support staff in a way that facilitates increased patient throughput.
- Creates/ develops clinical leaders through mentoring and by example.
- Oversees clinical schedule to ensure adequate coverage. Ensures clinical resources are scheduled appropriately. Collects, ensures accuracy of and approves timesheets from staff.
- Provides for the orientation, training, and periodic competency testing of clinical support staff as per WHI policy.
- Champions the essential electronic documentation for patient care and patient communication by understanding the, and being a subject matter expert, of the electronic health record system eClinicalWorks.
- Provides for the professional development and provision of resources to support the practice of quality patient care by RNs/LPNs and Medical Assistants.
- Ensures appropriate cross training of staff.
- Fosters collaborative working environment.
- Completes performance appraisals thoroughly and on time.
NURSING
- Maintains active registered nurse credentials and licensed in Washington, DC.
- Works with the clinical team to provide direct patient care.
- Assures the provision of teaching/learning opportunities for patients.
- Demonstrates complete competency when performing the staff RN role.
DAILY CLINICAL OPERATIONS
- Ensures all clinical staff understand their roles and responsibilities on the team.
- Proactively identifies clinical support issues that undermine patient flow and/or other clinic issues.
- Implements, monitors, and evaluates standard clinical policies and procedures.
- Works with the Practice Manager to monitor workflows.
- Ensures pre-visit planning occurs with each clinical team.
- Implements and ensures a process for daily team meetings and huddles with an integrated team.
PROBLEM SOLVING
- Takes initiative to solve problems and recommends improvements of department procedures and intra or interdepartmental processes.
- Effectively addresses employee relations issues, with assistance from Human Resources, in an appropriate and timely manner.
- Independently and effectively problem solves both patient and resource issues.
- Proactively identifies clinical support issues that undermine patient flow and/or other center issues.
QUALITY and RISK
- Demonstrates knowledge of quality improvement and performance improvement methods.
- Initiates continuous quality improvements efforts to improve staff performance workflow, efficiency and customer and staff satisfaction.
- Works with WHI leadership to develop reports on key quality and operational measures.
COLLABORATION
- Effectively collaborates with the Practice Manager and the Medical Director to identify and implement best practices.
- Works with the WHI Medical Director to lead a consistent forum to discuss and document the care of high risk/complex patients.
- Gathers and analyzes information for audits and special projects. Works with the Practice Manager to ensure that audits are completed in a timely manner.
- Works with the Medical Director and Practice Manager to ensure that clinical workflows are efficient and patient-centered.
- Communicates consistently and effectively with Leadership team to promote the successful functioning of the center.
REGULATORY
- Understands and adheres to the Patient Centered Medical Home (PCMH) model of care.
- Demonstrates superior knowledge of the requirements of other quality control standards.
- Participates in audits, surveys, and applications to acquire, sustain, and maintain accreditations, and recognitions.
- Handles routine patient complaints in an appropriate, timely and sensitive manner.
- Identifies and works to solve issues that are out of compliance surrounding Infection Control, OSHA guidelines and Environment of Care issues.
- Implements safety and emergency procedures throughout the center, consistent with WHI policy.
CARE MANAGEMENT
- Leads WHI’s care management program – Chronic Care Management, Transitional Care Management, and Medicare Wellness Visits.
- Identifies patients who require care management and care plans.
- Completes care plans for patients in care management.
- Assists patients with care coordination services and transitions of care.
- Collaborates with the Medical Director and Practice Manager on Lab and Referral tracking activities.
CUSTOMER SERVICE/PATIENT ENGAGEMENT
- Acts as a role model for clinical support staff in providing excellent customer service.
- Consistently displays professionalism in dealings with customers, staff and clinicians as observed by Supervisor.
· Direct patient care in case of a day/s of staff shortage.
- Document past medical and surgical history, as well as hospitalizations.
- Set visit agenda and expectations.
- Direct responsibility for the success of annual wellness visits.
- Responsible for injectables.
Communicate with external providers as needed to coordinate patient care.
Initiate and follow up on medication Prior Authorizations.
Referral Tracking
- Conduct and participate in referral tracking as instructed, including entry of Lab, Medication, PT, and referrals into the EMR system.
Skills and Qualifications:
- Comfortable serving under-represented and intersectional groups of individuals including Black, Indigenous, people of color (BIPOC); LGBQAI+; Transgender and other gender nonconforming individuals; persons with HIV; individual who are homeless or housing insecure; and other diverse communities.
- Arrive at work at 7:45 AM and share office space with another co-worker.
- Receive all recommended vaccines in a health care setting, including Hepatitis B, flu, and COVID-19. This is non-negotiable.
- Role requires in-person work only. This is not a remote work position.
- Minimum of two years’ experience preferred. Exceptional new graduates with strong references may be considered.
- Experience with eClinicalWorks is strongly preferred.
- Promotes a positive teamwork environment and excellent customer service.
- Excellent verbal and written communication.
- Experience in appointment scheduling, medical paperwork and supply management.
- Ability to multi-task.
- Able to practice at high standards of professionalism, confidentiality, patient safety and infection control.
- Spanish speaking a plus.
