Hometown Physician Services is a provider of in-home primary care to patients residing in assisted living residences and communities throughout areas in Wisconsin and Minnesota. Immediate part time opportunities available; with long term upward mobility and management opportunities as well. We are seeking to fill Nurse Practitioner positions to support & help lead care teams and provide on-site primary care to seniors in northwest WI (and some opportunity to assist in MN). This is an opportunity for a provider to help lead and guide care team plans & workflows, communications, clinical decisions and strategies, alongside involved collaboration, support and oversight by the clinical support team. Candidates should have interests in working with patients (especially seniors) with a range of physical and behavioral health needs and disabilities. Desired Talents and Qualifications:
- Graduate of NP or PA program from an accredited school;
- Must have and currently maintain valid NP or PA License (and DEA license) in Wisconsin (and Minnesota is a plus);
- Experience in primary care and proficient EMR knowledge is preferred; geriatrics and in-home care a plus;
- Proven ability to communicate and collaborate effectively with colleagues, patients and families;
- Strong interpersonal and customer service skills, in particular with seniors and disabled individuals;
- Must have a valid driver’s license and minimum state required liability auto insurance; reasonable travel to & from facilities is required;
- Demonstrated compatibility with Hometown's mission and philosophies to serve our communities' elderly and disabled individuals.
