We are looking for a Registered Nurse with the experience, skill, and initiative to work in a dynamic industry. A successful candidate will be caring, compassionate, and dedicated to providing safe personal assistance to clients of all ages. This is an individual who takes pride in assisting clients with their day-to-day activities, and we are looking for people who can anticipate clients' needs.
RESPONSIBILITIES:
- Use good judgment in handling emergencies.
- Direct and supervise therapy sessions.
- Assist with personal care tasks such as bathing, grooming, eating, etc.
- May transport patients to appointments.
- Prepare meals and snacks for patient.
- Complete daily reports.
- Conduct basic medical tests and procedures, including IV insertions and EKGs.
- Help with transferring patients between beds and rooms.
- Adjust treatments to patients' responses.
- Apply skillful handling techniques when moving patients between locations.

- Direct the nursing assistants to ensure quality care is provided at all times.
- Meet with nursing staff, as well as support personnel in planning the shifts services, programs, and activities.
- Prepare and administer medications as ordered by the physician.
- Complete assessments, care plans, treatments etc.
- Admit, transfer, and discharge clients as required.
- Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures.
- Ensure and maintain an atmosphere of warmth, personal interest and a positive environment.
- Positive communication with residents, families, staff and outside providers.
- Provides direct care and case management of assigned patients, including initial assessment and development of plan of care, as well as oversight and administration of prescribed medications and treatments.
- Consistently assesses and evaluates patient’s status and goals and revises the plan of care as necessary. Communicates adjustments to plan of care to all team members in the appropriate time frame to support consistent delivery of care.
- Consistently monitors and manages patient medications; maintains an accurate medication profile.
- Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician. Recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
- Provides and maintains a safe environment for the patient. Reports unsafe conditions, complaints, and incidents on behalf of patient as appropriate.
- Observes and supervises Home Health Aide to evaluate performance and quality of care.
- Accepts responsibility of assignment to perform a specialized procedure, such as IV therapy, wound care, phlebotomy when qualified with appropriate training and proven competency in the delivery of the therapy.
- Adheres to HIPAA laws and maintains patient confidentiality always.
- Other duties assigned by the case manager/clinical management, i.e., performing field supervisions of care team members as required by agency policies.
RN Documentation/Communication/Meetings:
- Completes and submits accurate, timely clinical notes including all OASIS documents, visit authorization requests, physician orders, and all other clinical documentation regarding patient’s condition and care provided in accordance with Natick Walpole VNA company policies.
- Communicates with physicians and other agencies providing nursing or related services on a consistent basis to ensure continuity of care and implementation of a comprehensive care plan. Acts as part of the interdisciplinary care team.
- Participates in scheduled clinical and internal/external case management meetings.
QUALIFICATIONS:
- Must hold a current Nursing License for The State of Massachusetts
- Prior Long-Term Care experience preferred
- Educational Requirements: Graduation from an accredited RN or program required.
- Experience Required: Three years of experience with one year of long term or sub-acute care experience required. One year of supervisory experience preferred.
- Professional Licensure and Certification Required: Current MA State RN Responsible for annual re-licensure per regulations. Compliance with yearly CEU requirements. Current CPR, Accucheck and IV Therapy certification.
- Must have strong clinical background and knowledge
- Must have previous experience as a Director of Nursing in a long term care facility
- Proven Leadership & Management skills
- Strong verbal & written communication skill
