POSITION SUMMARY
The Residential Service RN is an essential member of the Residential Services Management Team. Provide medical/nursing and safety services for assigned clients. Serve as a resource person providing health information, teaching and coordinating care with community resources for the Residential Programs.
The Residential Services RN exercises discretion and independent judgment to ensure quality of services delivered and operations are within regulatory compliance (OPWDD, OMH, DOH, OSHA) and agency guidelines.
ESSENTIAL RESPONSIBILITIES

Plan, develop, coordinate, and evaluate the delivery of specified health care services within the nursing scope of practice for volunteers, interns, workers, trainees, residents, and staff. In collaboration with the Safety Officer, the CQCO, and the agency leadership staff, monitor safe working and living conditions for all.
- Schedule and implement regular reviews with the Health/Nursing staff to assess the health and related safety needs of volunteers, interns, trainees, workers, residents, and staff; prepares specific courses to meet these needs; ensure the courses are implemented; ensure that competencies are validated by the responsible individual.

Monitor and review IRA resident and selected trainee medical files to ensure timeliness, accuracy, and regulatory compliance of the Health and Nursing Services Division’s record keeping system. Obtain as necessary reports, nursing care plans and case notes. Record in the electronic data base as required.

As needed, ensure the approved OMH and OPWDD Medication Certification class (AMAP) is taught to identified VTS, Work Center, and Residential staff. Review the content to assure it meets regulatory requirements and incorporates GHV policies and procedures.

Responsible for assuring that the annual pouring and re-certification of all Medication Certified Residential staff occurs.

Serve as a member of the Health and Safety, Intake/Termination and Incident Review Committees.

Function as a liaison with outside public and private agencies for health/nursing related matters.

Work cooperatively with appropriate designees to assess, plan and develop nursing care goals and necessary educational programs to accomplish the goals for GHV staff and clients.

Oversee the maintenance of client medical files for the OMH/OPWDD services.
- Coordinate and expedite the Worker’s Compensation cases for Gateway Hudson Valley’s workers and clients. Coordinate the provision of Infection Control Training to all new Work Center clients and workers and annually thereafter.

Work cooperatively with the VP Residential Services & Residential Directors, or his designees, to assess, plan and develop nursing care goals and necessary educational programs to accomplish the goals for GHV staff and residents.

Provide input to the VP of Residential Services regarding the operations of the Residential Department as related to Health Services, medication management, IRA care plans.

Participate in Residential Manager meetings monthly.

Schedule and conduct staff meetings for the Health/Nursing Services Division and Residential Department at regular intervals, including all Med. Certified staff.

On a monthly basis, complete the Medical Quality Assurance Review for the OPWDD-IRA residences.

On a bi-annual basis, complete a medication system audit for the OMH licensed residences.

Review the medical needs and oversight of individuals in the IRA program in accordance with regulatory requirements and coordinates related tasks and visits performed by the nursing staff. Assess residents, develop Nursing Care Plans, and delegate tasks as appropriate to residential staff.

Educate staff and validate competencies regarding delegated tasks, documents competencies. Provides feedback and recommendations to Residential Managers, Directors, and VP regarding observations. Provide consultation regarding needed management actions.
- Prepare semi-annual and annual nursing reports for each IRA resident.
- Is available to triage with residential staff, as needed.

Work cooperatively with the HR Manager, and any designees, in providing client and staff services according to regulations:

Coordinate the provision of orientation to new staff on HIV/AIDS and Infection Control, Post-Exposure, and Seizure Control.

Coordinate the provision of annual trainings to all Gateway staff on Infection Control and Blood borne Pathogen on a regular basis
-
Provide follow-up for Post Exposures including counseling and referral for further medical care and insuring completion of necessary paperwork and notification to the President and CEO or his/her designee.

Coordinate the provision of Bloodborne Pathogen Training for those clients (Custom Cleaning and Maintenance) who are designated to be in at-risk job classifications.
- Orders and monitors supplies for the Health Office for quantity and expiration dates.
- Disposes of Full Sharps Containers at the Kingston Hospital (Emergency Department)

Disposes and documents controlled substances with Staff Nurse. Responsible for disposal of non -controlled medication.

Dispense PRN Medications for clients with valid orders

Coordinates care of clients with nurses from other agencies.
REQUIRED QUALIFICATIONS
Education:
-
Associate’s Degree in nursing required
-
Bachelor’s Degree in Nursing (BSN) preferred
Experience:
- Two years’ experience in direct nursing care
-
Experience with working with individuals with developmental disabilities preferred
- Experience in writing and reviewing policies as well as experience in teaching and planning in-service programs desirable
Licenses / Certifications:
WORKING CONDITIONS
- Must be able to Sit or stand for 8 hours/day

Must be able to bend down or kneel occasionally (1-2 times/week)

Must be able to use hands, wrists, and fingers with full range of motion
-
Must be able to perform CPR in emergency situations and for regular recertification.

May occasionally be required to lift no more than 50 pounds (1-2 times per month)
