Our client is a leading community health organization dedicated to providing comprehensive healthcare services to underserved populations. They are committed to addressing infectious diseases within the community and are seeking a passionate and experienced Nurse Practitioner to join their team. Responsibilities Include:
- Conduct thorough assessments of patients with suspected or confirmed infectious diseases.
- Perform diagnostic tests and interpret results to determine the nature and severity of the infection.
- Develop and implement individualized treatment plans for patients, including medication management and follow-up care.
- Monitor patients' progress and adjust treatment plans as necessary.
- Educate patients and their families on infectious disease prevention, management, and transmission.
- Provide counseling and support to help patients adhere to treatment regimens and make informed healthcare decisions.
- Collaborate with infection control teams to develop and implement infection prevention measures within the community.
- Conduct assessments and recommend strategies to reduce the spread of infectious diseases.
- Work collaboratively with physicians, nurses, social workers, and other healthcare professionals to ensure comprehensive care for patients.
- Participate in case conferences and team meetings to discuss patient care plans.
- Maintain accurate and up-to-date patient records, including medical histories, treatments, and progress notes.
- Prepare and submit reports as required by regulatory agencies and funding sources.
- Participate in community outreach programs, workshops, and educational initiatives aimed at raising awareness of infectious diseases and prevention strategies.
Requirements:
- Must be a Nurse Practitioner licensed to work in NY.
- Minimum of 1 years of experience working in a clinical setting, preferably in infectious disease management.
- Strong knowledge of infectious disease prevention, diagnosis, and treatment.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a multidisciplinary team.
- Commitment to cultural sensitivity and providing care to diverse populations.
Please note- The annual base salary range is $100k to $150k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
