Hospice Services of Lake County is Growing and expanding We are seeking part time, per diem or on call LVNs to join our team of health care professionals. We offer competative wages and a supportive respectful work environment where employees feel appreciated and valued.
The organization's LVNs are responsible for providing direct patient care under the supervision of a registered nurse. This position also assists the RN Case Mangers by triaging current patients and communicating with physicians, pharmacies, and other members of the interdisciplinary team to provide quality patient care. The LVN follows an established plan of care, provides appropriate treatments, and works collaboratively with team members to achieve positive patient care outcomes.
Qualifications:
- Ability to proficiently use computer systems for electronic charting;
- Current California LVN license;
- Valid California driver's license;
- Reliable transportation for daily home visits;
- Successful completion of drug screen, physical examination and criminal background check at the time of hiring.

Competitive hourly wage, paid mileage, paid holidays and more. Normal working hours are from 8:00 am to 4:30 pm. Monday through Friday.
