7AM-3:35PM Monday-Friday
Responsible to establish relationships with local social service agencies, cultivate long-lasting relationships with clients and assist clients in the successful engagement and navigation of available services in the City of Pasadena.
ESSENTIAL FUNCTIONS
APPLICATION OF OPERATING MODEL – Apply Block by Block standard concepts, processes, practices and policies to daily operations to address local needs and concerns, focusing on the homeless population.
OPERATIONAL OVERSIGHT – Responsible for daily guidance and direction of outreach operations. Develop a custom deployment plan to maximize visibility and effectiveness of resources (personnel and equipment). Evaluate daily needs and adjust the standard deployment plan to best meet the needs of the client.
REPORTING – Utilize a combination of standard Block by Block reporting processes combined with processes developed specifically to update stakeholders on accomplishments and program effectiveness.
· Must be able to provide electronic and written reports of daily accomplishments and tasks completed.
· Must attend monthly, quarterly and annual meetings and report to board members and stakeholders using relevant data.
· Document all interactions in the field, in real time to be captured in the SMART System and utilize location service in order to provide walk paths showing routes.
· Develop a Top 10-20 list on most vulnerable, challenging or most seen people in the district in need of services and track progress in SMART.
· Participate in the monthly calls and meetings with Operations Manager and Project Managers for Outreach to review SMART system tracking and discuss challenges and successes.
· Participate in a monthly call with all Outreach accounts nation-wide on training, best practices, etc.
· Provide weekly and monthly SMART system reports to the customer to illustrate work being done in the district.
· OVERSIGHT – The Outreach Specialist is an important element of the programs visibility and is expected to active in daily outreach activity to include, Identifying those in need, Patrolling the district, Observing and Reporting, Providing hospitality and customer service, and Establishing new business contacts and following up with existing ones.
· ESTABLISH RELATIONSHIPS
· With the city, county, and state agencies that are in place to respond to the needs of the homeless and neediest of the community, strive toward a cooperative effort to make a positive impact on homelessness in the district.
· Attend essential monthly city and county outreach meetings to maintain contact with county service providers and stay up to date with new/amended policies and updates.
·FIELD CLIENT MANAGEMENT
· Develop personalized service plans tailored to each client and work with City of Los Angeles service agencies.
· Facilitate navigation of system services to insure clients receive assistance from initial engagement to soft hand- off to ongoing service provider.
· Follow through with clients in real-time. Calling services with client present to schedule next-step appointments.
· Facilitate transportation when needed for significant appointments to further progress in situations toward better goal.
· Receive and respond to calls in service district in real-time, during working hours. Proceed to site and deescalate reported issue when needed and problem solve client issues to reach desired outcome. Follow-up communication with caller on resolution of the concern.
· Utilize HMIS to document client interactions and track progress of services rendered from City of Los Angeles.

· Must maintain a positive, professional environment in full compliance with applicable laws, regulations, policies, procedures and overall Block by Block standards of expected professionalism.
ATTENDANCE
· Timely and regular, predictable attendance to meet expected productivity standards.
REQUIREMENTS
PHYSICAL ABILITIES- Must have the physical capacity to traverse continuously during an eight hour shift, with normal breaks (two fifteen minutes, and one thirty minute meal break).
·Must be able to work outdoors in all conditions with provided inclement weather gear.
COMMUNICATION – Must be able to speak, read, and write the English language in order to create basic reports. Must be familiar with City of Los Angeles ordinances and resources in order to effectively inform field clients, stakeholders and civilians when necessary.
REPORTING AND DOCUMENTATION- Familiarity with HMIS systems, how to facilitate access and maintain positive relationship.
EXPERIENCE- Verifiable prior case management experience; 3-5 years preferred.
