Position Summary
The TRIAGE NURSE Provides acute assessments that are age appropriate for Berks Community Health Center patient’s emphasis on prevention and health education. The triage nurse actively supports BCHC in its efforts to maintain designation as a Federally Qualified Health Center (FQHC) and as a Patient Centered Medical Home (PCMH). Participates actively in BCHC’s mission to provide exceptional patient-centered primary and preventive healthcare for all residents of Berks County, regardless of patient status.
Job Description
- · Provides age appropriate care to BCHC walk in patients.
- · Triage walk in patients complete nursing assessment and if necessary set up an appointment with a Provider.
- · Triage walk-in patients and transfer to ER those which are not appropriate for care in the health center setting.
- · Coordinate new patient ER appointments.
- · Reinforces with patients the importance and value of establishing a medical home with primary care provider for care of chronic problems. Refers BCHC patients or patients without primary care providers for routine follow-up care at the health center.
- · Utilizes age appropriate communication skills, interviewing and data collection skills.
- · Assesses and Interprets age-specific data, clinical and diagnostic findings accurately.
- · Prescribes diagnostic and pharmacologic interventions according to accepted standards.
- · Collaborates and consults appropriately with physicians, Clinical Manager, COO and specialists.
- · Collaborates and communicates with other health care professionals, as appropriate.
- · Refers patients to age-appropriate & needs-specific community resources.
- · Emphasizes health education. Provides individualized patient education and health counseling regarding prevention and health maintenance. Encourages patients and families to become educated consumers and to assume responsibility for their own health maintenance. Assures that patients/families are linked with community educational resources as needed. Provides (or provides for) group educational activities as appropriate.
- · Provides clear, concise, comprehensive documentation.
- · Integrates principles of continuous quality improvement into all aspects of practice.
- · Other duties as required.
Pay: $75,000.00 - $85,000.00 per year
