SUMMARY:
The Counselor is responsible for screening, assessing and providing a range of treatment services to patients. As part of a multidisciplinary team, the Counselor implements evidence-based treatments, including individual, couples, family and group counseling, case management, patient advocacy, psychoeducation and crisis intervention as needed.
ESSENTIAL FUNCTIONS (include but are not limited to):
- Conduct clinical screenings and assessments through observation, testing, interviewing and collateral information
- Develop, review and update individualized patient treatment plans in collaboration with multidisciplinary treatment team
- Coordinate all treatment services and provide evidenced-based clinical interventions
- Provide individual, couples, group and family counseling services using evidence-based treatment modalities
- Document all clinical interventions and communicate patient needs to treatment team in a timely manner
- Communicate with referral sources, outside providers, peer support networks and patient loved ones to coordinate care
- Collaborate with the Continued Care Specialist and other treatment team members to develop a comprehensive aftercare plan for each patient
- Complete utilization review notes and work with Sana's billing vendor to coordinate commercial insurance coverage as applicable
QUALIFICATIONS:
- Master’s degree in Social Work, Psychology or another licensure-eligible related field
- Active State of Vermont licensure as LADC, LPC, LCSW or LMSW and/or Psychologist (Master's Level) preferred or unlicensed and working toward licensure.
- Previous experience in the substance use disorder or behavioral health field strongly preferred
