This position is responsible for ongoing training and support of dialysis trained staff and patients/ family/caregivers providing dialysis treatments in the private residence.
PRIMARY RESPONSIBILITES:

Provides education and training to home staff and caregivers including but not limited to renal disease, use of home dialysis equipment and supplies, and dialysis emergencies.

Conducts home evaluations prior to initiation of patient treatments and as needed.
- Ensures that patient dialysis related orders are appropriately carried out within the established timeframe.

Evaluates patient satisfaction and quality of care provided.

Communicates with physicians at regular intervals to maintain appropriate desired patient outcomes.

Analyze patient physical status, laboratory data, and treatment records and communicate areas of concern with the patient’s physician and/or Medical Director.

Communicates all emergencies, patient life threatening complications, and related incidents to the nephrologist or their extender and the Area Manager.

Facilitates interdisciplinary patient care rounds/conferences and collaborates with the interdisciplinary care team in development and execution of the plan of care and achievement of goals.

Monitor and maintain patient dialysis medical records for appropriate documentation.

Promote effective and efficient utilization of supplies and resources.

Participate in Quality Assessment Performance Improvement (QAPI) through monitoring patient outcomes using the established methods from the time of admission through discharge.

Compliance with company policy and procedures in accordance with state and federal regulations for patient care and services (CMS, HIPAA, OSHA, or other governing agencies).

Performs similar or related duties as requested or directed.

Will require to be on-call at all times.
JOB REQUIREMENTS:
Education
-
BS in Nursing, preferred; or
-
AA or Diploma from School of Nursing.
Experience
-
At least 1 year RN experience, with minimum 3 months dialysis experience (18 months for Texas).
Skills
-
Excellent customer service and interpersonal skills;
-
Good observation, critical thinking, and documentation skills;
-
Previous experience using home dialysis equipment;
-
Basic to moderate PC skills;
-
Familiarity with use of EMR systems
LICENSES, CERTIFICATIONS:
-
Current RN State License required in state(s) of oversight;

Current BLS certification required; and
-
Obtain dialysis certification within one year of employment

Current driver’s license
PHYSICAL REQUIREMENT Able to sit and work at computer for extended periods of time;

Able to sit and work at computer for extended periods of time;

Walk, sit, lift 35 pounds;

Must be able to identify and differentiate colors; and
-
Must be willing to travel to patient’s homes as needed.
Salary Range: $32.00 - $55.00 per hour. **BASED ON EXPERIENCE.**
